        Introduction: Preventing the spread of drug resistance is one of the most important issues in society. Escherichia coli is one of the most common bacterial agents isolated from the urinary tract and nosocomial infections. Treatment of infections due to it is difficult due to the acquisition of resistance genes. This study aimed to investigate the phenotypic and genotypic pattern of antibiotic resistance in clinical uropathogenic strains isolated from diabetic patients. Materials and methods: A total of 51 E. coli isolates from urinary tract infection in diabetic patients obtained from clinical, were used in this study. Isolates were confirmed by chemical tests and molecular techniques based on tracking of the 16srRNA gene. Antimicrobial resistance assessment of isolates was done using molecular methods base on (qnrA, tet A, tet B, aac (3)IIa, sul1) and disk diffusion. Results: Most resistance r to ampicillin (66.66%) and the lowest resistance to Nitrofurantoin (1.96%) were reported. The frequency of tet A, tet B, qnr A, sul 1 and aac (3) IIa genes reported 68.62%, 64.7%, 29.41%, 39.21%, and 29.41% respectively. The statistical analysis shows a significant relationship between resistance to the antibiotic tetracycline and the gene tet A, tet B statistically. Conclusion: Early detection of resistant strains to select the most appropriate treatment options is essential to prevent the spread of resistance. It is suggested, as treatment for urinary tract infections is important, so to prevent drug resistance and treatment failure, it should be done according to the resistance pattern in the region.         Due to the innate tendency of human beings to interact with nature and the significant effects of communication with nature in the human soul and body, human beings have always sought to create a logical and appropriate relationship between indoor space and outdoor nature. The quality of natural environments not only is not limited to the health and well-being of individuals, but it also provides their spiritual and psychological needs, paves the way for development and progress, identity formation, and extraction of various environmental experiences such as fun, vitality, and mind retrieval. Healing is a term that repeatedly seeks to enhance human comfort in the natural landscape. It is a quality that encompasses both body and mind, and generally refers to the spatial nature that cares about human health and improves it directly affects the human psyche. There are two common types of green space with healing properties: sensory gardens, which offer passive health benefits, and therapeutic (physical) gardens provide active relief. Along with the formation of human societies, the phenomenon of participation has emerged in order to strengthen social relations regarding the diversity of attitudes and schools. The definition of social sustainability refers to four key elements: social justice (meaning equal opportunities and human rights for all people, both now and for future generations), social solidarity (meaning empathy), participation (meaning providing opportunities for each individual in society so that each person can improve their quality of life) and security (meaning providing security in livelihood and Against natural hazards). Nowadays, stress is the cause of mental disorders in society. Due to the fact that a large number of people face a lack of medical centers appropriate to their moods, their needs, and using concepts related to social sustainability, the design of medical spaces can be solved by involving the audience according to their wishes. In the present study, by using qualitative strategy and using library studies, and applying the description-analysis method and effective components of the healing garden in improving patients. To determine the participation approach, the relationship of each component with the type of patient participation was identified through study and conclusion, which identified four types of participation approaches. First, in the planning stage, by selecting operating groups with an interpretive approach based on evidence-based design, the wishes and needs of patients are identified. Then, in cases of components that need to get enough information from patients, information is obtained through various tools such as questionnaires, interviews, etc. In the design stage, by reviewing the initial solutions and ideas obtained from the planning stage, using the regionalism approach to the indigenous cultural and historical features of the region, along with the universal design, which refers to the free and flexible design, to all tastes. The right answer will be given and the final product will be obtained with collective ideas. In all these approaches, the type of patient participation due to their specificity and personal limitations of individuals is indirectly discussed in the plan. Manuscript profile

        Introduction: the guardian less patients are considered as hospital debtors and their financial burden is imposed on hospitals, this research has been conducted to evaluate the expenses of guardian less patients in selected hospitals of Shiraz University Medical Science. Research Method: this cross-sectional study has evaluated the financial burden that guardian less patients imposed on Shiraz selected Hospitals in the second half of the year 1393. The data was gathered through review of patient invoice and medical records using a self-administrated check list. The gathered data was analyzed using, ANOVA Test in SPSS21 software. Results: the findings indicated that 183 guardian less patients were discharged from hospital in the study period. Most of the patients were men (%58/6), married (%52), native (%77/3) and had health insurance (%61/5). The total expenditure of these patients was 8535929734 IRR which 5184119454 IRR was reimbursed by insurance organization. 3330828022 IRR was not compensated. Uncompensated cost was statistically different in different hospital and based on type of who referred patient. Conclusion: Considering financial issues which hospitals face, using various interventions to decreasing uncompensated cost will be useful. Employing social workers who are professional in connecting with social support organizations and also improving population coverage of insurance plan will be helpful. Manuscript profile

        Introduction: High level of cost for inpatient services made a reasonable reason for Iranian Ministry of Health and Medical Education in order to implement a reform called: “Health transformation plan”; it has been aimed to decrease patient out of pocket. High prevalence of cardiovascular disease and high level of cost for these services have significant contribution to increase health system expenses and patients payments. This study is aimed to investigate and compare the portion of cost payers’ contribution for inpatient services provided to cardiovascular patients before and after health system reform. Methods: This descriptive- analytical study was conducted on 601 CABG patients referred to Al-Zahra Heart hospital in Shirazbefore and after Iranian Health transformation plan implementation. Required data gathered by hospital bills. data were recorded in the data sheet then entered excel software for statistical analysis, and they were analyzed by SPSS software and Mann-Whitney U statistical test for non-parametric situation.  Results: The mean of patients out of pocket after “Health transformation plan” implementation for CABG cases decreased significantly (p<0.001) from 10,649,295 to 6,971,268Rials. There was a significant increase for mean of basis insurance share from total cost of a unit CABG (p<0.001) as it rose from 41,847,802 to 94,782,096Rials. Conclusion: Having implemented the “Health System reform” in Iran leading to decrease patient out of pocket for operations significantly, there is a massive payments and costs must be covered by Iran Ministry of Health which imply huge costs and duties to this organization.         Introduction: Identifying the trends and causes of mortality can be a base for health-care policy-making and planning in society. This study aimed at identifying the causes of the mortality of hospitalized patients based on International Classification of Diseases. Methods: This retrospective study was a descriptive-analytical one. Research population included all inpatients died in hospitals under supervision of Semnan University of Medical Sciences during a 5-year period (2011-2016). Sampling was done as the senses and data were collected by a checklist extracted from death registration system of the Treatment Deputy of the university. Data were analyzed in SPSS by applying statistical approaches in the significant level of p≤ 0.05. Results: 8,270 hospital mortalities were registered during the period. 6,370 (76.3%) of dead patients were burghers and 4,450 (53.8%) were male. The most frequent causes of the mortality were cardiovascular diseases (3,201, 38.7%), cancers (1,287, 15.5%), and respiratory diseases (865, 10.5%). There were significant differences among the mortality rates according to patients' gender, residences, and age ranges (p< .001). Conclusion: Chronic and non-communicable diseases were the main causes of the mortality. Health policy-makers can decrease in these diseases by applying screening programs, empowering people and endorsing regulations on safety life style and so on.         Introduction: Considering the importance of the effect of chronic renal failure on various aspects of patients' life, the purpose of the study was to determine the factors affecting the quality of services provided to hemodialysis patients. Methods: The research was carried out using a mixed method. Samples were targeted at the qualitative stage and included 24 experts from hemodialysis section from 6 provinces of the country. In the quantitative phase, 150 employees in the hemodialysis department were examined. The information gathering tool in the first step was a library and in the second step was a questionnaire. The researcher-made questionnaire was designed with 6 elements; validity and reliability of the questionnaire were also evaluated using goodness of fit. The analysis in the descriptive statistics section included frequency, percentage, mean and standard deviation, and was used in the inferential statistics of exploratory and confirmatory factor analysis. Exploratory factor analysis was performed using SPSS 21 software and confirmatory factor analysis through Laser Level software. Results: Six factors influencing the quality of hemodialysis services were identified, and based on the importance were: human resources (0.866), management (0.874), information technology (0.749), equipment (0.731), facilities (0.683) and physical space (0.628). The fittest indices also showed that effective factors have suitable fit and can be mentioned as factors affecting the quality of hemodialysis services in Iran. Conclusion: Due to the low quality of life in hemodialysis patients, appropriate strategies should be taken to prevent the loss of resources and improve the patients' life quality by using the result.         Introduction: Health literacy refers to the ability of individuals to acquire, analyze and understand the information and basic health services they need to contribute to their health issues and make the right decisions. Today, health literacy is one of the pillars of community health. The purpose of this study was to provide a model for assessing the effect of health literacy and self-care measures on the quality of life of cardiovascular patients after open heart surgery. Methods: This study is an applied research and based on the method of data collection is a descriptive-survey type that the statistical population of the study was all patients who had open heart surgery at the Tehran heart center and among them, 138 people were selected by available sampling. Data gathering tool was a researcher-made questionnaire whose validity was confirmed by face validity and convergent validity and reliability of the questionnaire was confirmed by Cronbach's alpha. Structural equation method was used to analyze the collected data. Results: The results of hypothesis testing showed Health literacy with path coefficient (0.685) has a positive and significant effect on self-care measures, Health literacy with path coefficient (0. 350) has a positive and significant effect on patients' quality of life, and self-care measures with path coefficient (0.418) have a positive and significant effect on patients' quality of life. Conclusion: Improvement of health literacy and self-care behaviors in cardiac patients after open heart surgery can improve the quality of life of these patients after surgery and reduce complications after surgery. Therefore, the main purpose of this study was to identify the major factors affecting the preference of medical centers in other cities by the native patients of Sarab. Results: The results showed that 9 factors of capacity, cultural-social, diversity of health centers, human resources, mental image and public trust, managerial, behavioral, services, and medical equipment were identified as the main factors influencing the preference of medical centers in other cities. Conclusion: Based on the results of this study, it is suggested that the planners of Sarab city health centers consider the factors identified in this study in order to prevent local patients from leaving the city for treatment.         Introduction: Hypertension is the third leading cause of death in the world. One of the ways to control chronic diseases is to increase patients' awareness, which requires improving their health literacy. For this purpose, in this study, a health literacy model for hypertensive patients in Shahid Rajaei Heart Hospital is presented. Methods: This research is applied in terms of purpose and in terms of survey method and a combination of qualitative and quantitative methods. The statistical population of the qualitative section is all experts in the field of health and treatment, and in the quantitative section the staff has a bachelor's degree or higher from Shahid Rajaei Heart Hospital. In the qualitative part, after conducting 23 theoretical saturation interviews, in the quantitative part of the research, 122 employees of Shahid Rajaei Heart Hospital were selected by counting method. In addition to library studies, the field method has been used through interviews in the qualitative section and questionnaires in the quantitative section. The validity of the questionnaire was confirmed by CVR and its reliability was confirmed by Cronbach's alpha. In order to analyze the data, open coding method in qualitative part and structural equations in quantitative part have been used. Results: The results showed that basic literacy with a coefficient of 0.923, cognitive literacy with a coefficient of 0.648, social literacy with a coefficient of 0.585 and cultural literacy with a coefficient of 0.483 are of great importance in the health literacy of hypertensive patients in Shahid Rajaei Hospital, respectively. Conclusion: Based on the findings of this study, it can be concluded that the existence of 4 basic literacy factors with reading and writing components and knowledge, cultural literacy with attitude components, values and norms, cognitive literacy with application components, evaluation, comprehension And access, and social literacy with components of communication and interaction can affect the health literacy of hypertensive patients. The new man by feigning ignorance of the death and dying, try to make it far from him or her. Medical centers and burial institutions support this attitude and attempt to show that in human’s life the death does not enjoy of any essential reality. Elisabeth Kubler-Ross was one of the thinkers that believe that this attitude should to be change and that the denial of death is placed by its acceptance. Kubler-Ross begun her work by the book On Death and Dying (1969). In her book, by appealing to empirical observations, she claims that terminally ill patients when are faced with the reality of their impending death, through a mechanism of five stages can arrive at the acceptance of their death. These stages are denial, anger, bargaining, depression, and eventually the acceptance. This theory or model from the time of its appearance was the subject of many critiques such as not making use of previous researches done on the topic of death and dying, ambiguity in the five stages, limitations of method of gathering information, not to pay attentions to individual differences and also to the impact of environment on the person.         Background and Objective: Urinary tract infection (UTI) is one of the most common hospital infections acquired by colonization of uropathogenic E. coli in host musocal epithellium and damages to host tissue. The ability to constitute biofilm plays an important role in virulence of the bacteria. Materials and methods: In this research 51 E. coli strains isolated from diabetic patients having UTI symptoms collected and approved using biochemical tests and molecular technique. In vitro biofilm formation by these isolates was determined using the 96-well microtiterplate test. And the presence of fimH, pap, sfa and afa virulence genes was examined. Results: Among 51 Ecoli isolates, 46 (90.16%) were able to make biofilm. The frequency of fimH, pap, sfa and afa genes in isolations having the ability to create strong biofilm reported respectively 90%, 80%, 80% and 10% reported, The frequency of fimH, pap, sfa and afa genes in isolations having the ability to create weak biofilm reported respectively 83.33%, 75%, 15% and 41.66% reported. Discussion: The results of this study showed that the prevalence of fimH and pap genes among UPEC strain is high.         The main purpose of this research were to study health- promotion lifestyle (HPL) of female rheumatoid arthritis patients in comparison with healthy women and to determine the relationship between demographic factors (including age, disease duration, income, and education) and lifestyles of these patients. With available sampling in 2011(1389s.c) in the city of Tabriz, 105 female rheumatoid arthritis patients and 105 healthy women participated in the research. Research instruments included a researcher made demographic questionnaire and Health Promoting Lifestyles Profile: || (Walker, Sechrist&Pender,1987). The data were analyzed by applying t- test for independent groups and multiple analyses of Variance. The data indicated that the healthy women had higher overall scores on the total HPL and six subscales than rheumatoid arthritis patients. Furthermore, based on t- test result, significant differences were observed between the overall rheumatoid arthritis patients HPL scores and healthy females except in stress management dimension(p < 0.01). According to the results of regression analysis, 0.28 percent of the variance in the patients PHL can be explained by the demographic factor of education. These findings, it may be suggested that professionals encourage rheumatoid arthritis patients to practice in health- promo/ting behaviors with appropriate interventional and education methods.         The purpose of this study was examination of effectiveness of logotherapy by group method on increase of life expectancy and health of female teenager major thalassemia patients of Ahwaz city. Statistical community of this research consists of the whole major thalassemia female patients 15 to 18 years old referring to Abouzar hospital in 1388 spring. Pre-test practiced for 110 Persons of clientele and among them 46 persons which obtained the lowest score in GHQ28 Public health test which is b score and the lowest score in Millers life expectancy which is a standard deviation under average. were selected as statistical samples and then sample persons were replaced randomly in two "23 persons groups" as experiment and control groups. Measurement tool of this study was Miller's life expectancy and general health questioner (GHQ28) test and research plan was type of pre-test and post-test with control group and a follow up session.After that, during 10 "one hour sessions", logotherapy techniques were educated to experiment group, and then the post-test was practiced to the both groups (experiment and control). And in the next step, after one month Miller's life expectancy and general health GHQ28 test (follow up test) was practiced again to measurement of presented educations permanence. Statistical method of variance analysis, were used for analyzing the data. The results of the study showed that logotherapy education can increase the life expectancy and general health in major thalassemia patients (P < 0.0001)Also, Permanence measurement of the research (P < 0.0001) showed the same result.         The purpose of this study was to evaluate the effectiveness of schema therapy on negative affect and perceived stress in coronary heart patients. The research method was quasi-experimental with pre-test and post-test with control group. The statistical population of the study consisted of all coronary heart patients referring to Jamaran hospital in Tehran in 2018 which were selected by purposeful sampling method. 30 of them were selected and randomly assigned into two groups of experimental (15 subjects) and control (15 subjects). Data were collected using the Denollet Negative Affect Inventory (1998) and Cohen et al. Perceived Stress Scale (1983). After the pre-test, intervention (Schema Therapy) was given to the experimental group in 10 sessions with duration of 50 minutes. At the end of group Schema Therapy program, post-test was taken from both groups. Data were analyzed using multivariate covariance test. The findings showed that by controlling the effect of the pretest between the means of the posttest for the experimental and the control group, there was a significant difference between the variables of perceived stress (F=17/897) and negative affect (F=16/082) at P&lt;0/01 level. Based on the results of the present study, the application of this method can be useful in planning therapeutic interventions in cardiovascular. Manuscript profile

        Present research aimed to investigate the effectiveness of training based on forgiveness on resilience and marital burnout of patients with multiple sclerosis. This study was a quasi-experimental with a pre-test and post-test design with a control group. The research statistical population was all women with multiple sclerosis, member of the association for the support of multiple sclerosis patients of Tehran city in 2017 year. Totally 30 people selected through available sampling method and randomly assigned to two equal groups (each group 15 people). The experimental group trained 8 sessions of 90 minutes undergoing the forgiveness training. Groups completed the questionnaires of resilience (Connor &amp; Davidson, 2003) and marital burnout (Pines, 1996) in the pre-test and post-test stages. Data was analyzed with multivariate analysis of covariance method. The findings showed that training based on forgiveness was effective on forgiveness on resilience and marital burnout of women with multiple sclerosis. In the other words training based on forgiveness led to increase resilience and decrease marital burnout of women with multiple sclerosis (P&lt;0/05). As a result, training based on forgiveness is an effective method to improve the resilience and marital burnout of women with multiple sclerosis. Manuscript profile

The statistical population of this study included all cancer patients and normal people in Ahvaz city who referred to health centers. Of these, 80 were selected by available sampling method. The research project was causal-comparative. The instruments used in the Sense of Coherence Questionnaire (Aaron Antonovsky, 1987), the Pennsylvania State Scale (Maeir et al., 1990) and the general self-efficacy beliefs questionnaire (Schweurzer and Grosselm, 1995). Data analysis was performed by using one-variable and multivariate analysis of variance analysis. Findings showed that there is a significant relationship between sense of coherence, concern and self-efficacy between cancer patients and normal people.         Purpose: The aim of this study was to the Prediction of Health Promoting Lifestyle Based on Sense of Coherence, Psychological Flexibility and Impulsivity in Cardiovascular Disease in patients Obesity. Methodology: The research method was correlation using structural equation modeling. The statistical population of this study consisted of all cardiovascular patients with obesity in Tehran in 2018-19, of which 220 people were selected as a sample by stepwise cluster sampling. The research tools were: Walker et al.'s (1987) health-promoting lifestyle, Kimiyaei, et al. (2013), Bond et al.'s flexibility (2011), and Bart's impulsivity (1995), all of which have acceptable validity and reliability. The analysis obtained by the implementation of the questionnaires was performed through the Spss-V24 software at a significance level of 0.05 and using the step-by-step regression test. Results: The findings showed that a sense of cohesion, psychological flexibility and impulsiveness predicted a significant health-promoting lifestyle in obese cardiovascular patients (P &lt;0.05). Conclusion:&nbsp; Based on the research findings, it is possible to develop appropriate and effective educational programs by identifying the factors affecting the quality of life and mental health of individuals.         Purpose: Hepatitis is a major health problem in the world and is one of the ten leading causes of death in humans. This disease is an important cause of cancer and liver disorders and causes disabilities in people. The aim of this study was to determine the role of optimism in the relationship between social support and treatment adherence of hepatitis patients in Zanjan. Methodology: The method of this research was correlational. The statistical population of the study included all patients with hepatitis referred to the Comprehensive Health Center No. 5 in Zanjan in the first half of 2019 were 999 people, who 210 patients was selected by accessible randoming in the study. To collect the required data, McLean (1993) Ambiguity Tolerance Questionnaire, Sherborne &amp; Stewart (1991) Social Support Questionnaire and Moriski et al. (2008) treatment adherence questionnaire were used to collect the required data. Also, Pearson statistical tests and multiple regression in Spss software version 25 were used to analyze the data. Significance level was considered 0.05 for all tests. Findings: The results showed between social support with treatment adherence (P&lt;0.001, r= 0.360), between social support with optimism (P&lt;0.001, r= 0.329) and also between optimism. There was a direct and significant correlation with treatment (P &lt;0.001, r= 0.349). The results also showed that the effect of total social support on optimism was significant (P&lt;0.001, &beta;= 0.442). The partial mediating role of optimism in the relationship between social support and treatment adherence was significant (P&lt;0.05). Conclusion: In general, the results of this study showed that social support with partial mediation of optimism was significantly associated with treatment adherence of hepatitis patients. To accomplish the stated goal, 79 were selected by means of proportional sampling procedure from Hazrat Fatemate Zahra Hospital (Mazandaran Heart Center) in Sari City. The Social ReadjustmentRating Scale (Holmes &amp; Rahe, 1967) and The Depression, Anxiety and Stress Scale) Lovibond &amp; Lovibond, 1995) were administered on them. Analysis of data using a multiple regression analysis revealed that the magnitude of depression can be predicted from stressful life events in cardiovascular patients. Moreover, data revealed that patients that had higher in stressful life events were higher in depression.         Actual physical illness may be a major vulnerability factor in promoting health anxiety, and there is evidence of this from general medical setting to determination the health anxiety in Iranian patients that refer to medical clinics, this study was done. In this descriptive- comparative study, sample contains 212 patients attending to 6 clinics at two general hospital of Shahid Beheshti university of medical sciences were evaluated.&nbsp; After informing consent, the health anxiety inventory (short form) and demographic question air were administered to patients. We used one way ANOVA test to determine differences in outcomes among groups. The data were analysed by SPSS 19 software. The present study included 65 (30.7%) male and 147 (60.3%) female with mean age 38.64 (SD=12.7). There were no significant difference between two sexes in health anxiety scores. The results of one way ANOVA test for comparison among six groups showed significant difference in health anxiety in patients. Our results indicated significant difference in health anxiety scores in medical patients. Manuscript profile

        The purpose of this study was to determine the effect of self-care on depression and quality of life in diabetic patients in the health center of Qom martyrs. A semi-experimental study with pre-test, post-test and control group. The statistical population of all people with diabetes was referred to the health center of Qom martyrs' martyrs in 2017. Among them, 50 were selected by simple random sampling method ( 25 for the experimental group and 25 for the group Control ) were selected. The instruments used were Beck Depression Inventory and World Health Organization Quality of Life questionnaire before and after implementing the self-care training method on participants, and data were analyzed using descriptive statistics ( mean and standard deviation ) and inferential statistics ( Covariance analysis ) were analyzed. The study was applied in terms of purpose and descriptive-correlational in terms of method, where the correlation between each variable was analyzed through modeling and path analysis. The population was all outpatient and hospitalized cardiac patients from April 2018 to June 21, 2018 who were admitted to Farshchian Hospital wards and outpatients admitted to the heart clinic as well as the offices of cardiologists in Hamadan. Then 250 subjects were selected randomly using convenience sampling method.The research tools consist of quality of life questionnaires (Varsharbun, 1992), hardiness questionnaire ( Kobasa , 1979), five factors of the Neo character (Costa and McCraey, 1992) and the Life Expectancy Test (Miller &amp; Powers, 1988). SPSS-22 and AMOS-22 were used to analyze the data. In general, using the structural equation modeling technique and using the Amos22 software, the research hypotheses are tested.&nbsp; The results showed that the coefficients of life expectancy and personality traits had a significant correlation, with standard coefficients of 0.249 and 0.386, with hardiness of cardiac patients.         Adolescents&rsquo; delinquency along with various social, economic, mental and physical problems is important for Iranian society. As adolescents are the active population so, their physical and mental health must be taken into serious consideration. Accordingly, the present study aims to investigate the role of media on the adolescents&rsquo; delinquency. It is a type of quantitative study using a questionnaire to collect the required data. The population consists of two groups of the Correction and RehabilitationCenter patients of Shiraz city and a number of non-patient citizens aged 13-18 years. Through the stratification sampling method 55 patents and 55 non-patients were selected. The data were analyzed via the SPSS software. the results shows that getting influenced by negative characters of movies, using satellite for entrainment and watching CDs negatively associate to delinquency on both groups. Also, using the internet and watching thrillers revealed a significant relationship with delinquency of the patients. However, no significant association exists between watching movies and scientific TV shows among non-patients.         Great attention should be paid to planning and scheduling surgeries in the operating room which is the most sensitive ward in the health context in terms of cost and specific sensitivity due to its association with the life and death of individuals. In this case, reusable sterile equipment and devices are crucial issues because the hospital or nosocomial infections result from insufficient sterilization of these instruments. Therefore, sterilization of reusable medical devices is a necessity in the operating room to prevent possible infections. This study solves the integrated operating rooms and sterile section planning problem to minimize the total costs of sterilization, surgery postponement, and performance. This study also minimizes the completion time of surgery considering nondeterministic operating times and emergency-elective patients. In the real world, surgery time may be nondeterministic based on the conditions of the patient, surgeon, equipment, and instruments; hence, it is valuable to find a robust solution for planning under such circumstances. After presenting a bi-objective mathematical model for this problem, an improved epsilon constraint method was used to solve problems with small dimensions, and two metaheuristics NSGA-II and NRGA were developed for large dimensions regarding NP-hard problems. These two algorithms were analysed in terms of five indicators. The results indicated the superiority of the NSGA-II algorithm over NRGA to solve such problems. Manuscript profile

        The effectiveness of cognitive-behavioral family therapy in the treatment of patients with psychosomatic musculoskeletal pain was examined. Applying an A-B single-subject design, three patients (two males and one female) with psychosomatic musculoskeletal pain in the areas of lower back and neck, referred to a private, psychological clinic, were selected as the sample. Clinical interviews, MMPI and ScL--R tests were done to measure psychological problems. Family therapy was done and post test results showed the effectiveness of cognitivebehavioral family therapy by promoting patients&rsquo; skills of problem solving and communication, effective coping and self assertion.         Introduction and Objectives: Escherichia coli and Klebsiella pneumoniae are the most agent of urinary tract infection (UTI) and prevalence Extended-Spectrum Beta-lactamase (ESBLs) in these bacteria due to spread of antibiotic resistance and mortality and morbidity in patients. The best manner for control of ESBLs in bacteria, are inhibition of spread these bacteria and use of standard method for recognizes ESBLs producer strains. Subject of this study was comparison frequency of ESBLs in Escherichia coli and Klebsiella pneumonia strain in UTI acquired patients with phenotypic test. Materials and Methods: This cross-sectional search was performed in Azzahra and Shariaty hospitals during of 2008-2009 years in Esfahan, according to statistical formula randomly selected 91 samples from urinary infections. Bacterial identification was performed with microbiological methods, ESBLs production was performed with screening and confirmatory test and survey antibiotics resistant pattern was performed with Kirby method. Results: Frequence of ESBLs in E.coli and K.pneumoniae strains was respectively 47.97% and 41.66% .According to antibiogram result respectively 59.2%, 54.9%, 30.3%, 27.8%, 19.5% and 16.7% of E.coli strains were resistant into Co-Trimoxazole, Nalidixic acid, Ciprofloxacin, Gentamicin, Ceftazidime and Nitrofurantoin and respectively 75%, 50%, 40%, 44.5%,37.5%, 37.5%, 22.3% and 0% of K.pneumoniae strains were resistant into Ampicillin, Co-Trimoxazole, Nitrofurantoin, Ceftazidime, Amikacin, Cephotaxime, Imipenem and Ciprofloxacin. Conclusion: The result showed high frequence of ESBLs, so&nbsp; antibiotic resistant in isolated bacteria from hospitalized into out patience's that represent high spread&nbsp; antibiotic resistant strains in hospitals.         Background &amp; Objectives: Pseudomonas aeruginosa is an important cause of nosocomial infections in burnt patients. Antibiotic resistance through various mechanisms is one of challenges for treatment of these patients. The mex efflux pumps play a vital role in the development of multiple resistances to antimicrobial drugs. This study aimed to analyze the prevalence of genes responsible for efflux pumps mexA-B-oprM and to investigate their phenotypes in the isolates. Materials &amp; Methods: In this cross-sectional study, 250 swabs were obtained from wounds of the patients suffering of burns levels 2 and 3 who admitted in Ghotbodin Shirazi hospital, Shiraz. Presence of P. aeruginosa isolates were confirmed by biochemical tests and PCR. Drug susceptibility, the phenotypic activity of efflux pumps and the presence of mex A, mex B genes were determined using disk diffusion method, cartwheel method and PCR methods, respectively. Results: In this study, 26.40% (66 cases) of patients with burn wounds were infected with P. aeruginosa. These bacteria were resistant to all antibiotics tested except for colistin. Totally, 66.66% of the isolates (44 cases) had an efflux pump, among them 42.92% and 87.87% of the isolates carried mexA and mexB genes, respectively. Conclusion: Our finding showed that genotypic method is very accurate and reliable than phenotypic methods for detection of efflux pumps in clinical isolates of P. aeruginosa. Due to presence of the efflux pump genes in more than 90% of the P. aeruginosa isolates, analysis of the presence of these genes is very important for suggestion of an effective treatment model for the patients with bacterial infection.         &nbsp; The present study was conducted to investigate the efficiency of Memory Specificity Training intervention on improving problem solving skills and autobiographical memory in patients with Bipolar disorder with psychotic traits. It is done by quasi-experimental method. Among all patients with bipolar disorder with psychotic features of Sarai Ehsan Complex (Chronic Mental Patients Care Center) in the suburbs of Tehran, a sample of 30 people (15 people to perform Memory Specificity Training and 15 controls) were selected by purposive sampling. Biographical Memory Test, Problem Solving Questionnaire (SPSI-R), and Memory Specialization Training (MEST) treatment protocol were used to collect data. Research data were analyzed by multivariate analysis of covariance, univariate covariance and one-way ANOVA. The results showed that there was a significant difference in problem solving skills and biographical memory of patients in the intervention group of memory specialization training and the control group. Therefore, we conclude that the intervention of memory specialization training is effective in improving problem solving skills and biographical memory of patients with bipolar disorder with psychotic traits and demographic factors can play a role in problem solving skills and patients&rsquo; biographical memory.         The aim of this study was to identify and understand the social needs of caregivers of cancer patients in order to provide the necessary information to design educational and psychological interventions based on the needs of these caregivers. This research was conducted in a qualitative manner using the facet theory approach in 1399. Participants are caregivers of cancer patients in Tehran. Data were collected through semi-structured interviews with 21 caregivers and using theoretical and Purposeful sampling to achieve information saturation. Based on the coding of the texts of the interviews and observations, their needs and classifications were extracted. The social support needs identified can be classified into three categories: emotional, instrumental, and information. The sub-categories extracted from these results are: "Need for specialized centers and support organizations","Need for job support"," Need to connect with the community "and" need for medical information and mental health ". The social consequences of cancer are significant for the caregiver. In terms of the need to connect with the community, it seems that separating the caregiver from the community is serious and even after completing the care period, it is difficult for the caregiver to return to the community. Educating people in the community is useful for better understanding caring conditions. Having medical information and mental health helps the caregiver to create a sense of control, so it is necessary to create awareness among health care professionals, and finally job support was identified as one of the important factors to meet the caregiver's tool needs. From the statistical population using purposive sampling method and based on the score of the relevant questionnaires in the pre-test, 30 people were selected as the sample. These 30 people were randomly selected in 2 groups of 15 experimental and control groups, the experimental group received self-compassion training and the control group is on the waiting list. Also, analysis of covariance was used to analyze the data and SPSS24 software was used to accelerate the results. The results showed that self-compassion training significantly reduced alexithymia and increased self-care behaviors in patients with MS. In other words, alexithymia in patients with MS in the experimental group were significantly lower and self-care behaviors were higher than the control group. As a result, it can be said that self-compassion training is a good approach to reduce alexithymia increase self-care behaviors in patients with MS. Manuscript profile

        This research investigated the effect of the body of the hospital spaces on stress in neuropsychiatric patients. From the statistical population, which includes all male patients (veterans and non-veterans) hospitalized in Isar Neuropsychiatric Hospital of Ardabil, a sample of 50 people was selected by available sampling during May to July 2019. For data analysis, multiple linear regression model was used, with 10 physical indicators of patient room architecture as independent variables and patients' stress level as dependent variable. The independent variables were evaluated by architectural experts and the 21-question DASS-21 questionnaire was used to calculate the amount of patients' stress. Natural light, control of disturbing noises, "variety in the shape, color and material of furniture" and the position of the patient's bed compared to other beds, furniture and openings respectively had the greatest impact on reducing patients' stress. In the case of veteran patients, artificial light, natural light, distance to the toilet, "the position of the patient's bed in relation to other beds, furniture and openings", control of disturbing sounds, and "variety in the shape, color and material of furniture" have the most impact on Veteran patients had a reduction in stress. In the case of non-veteran patients, only "natural light" had a significant effect on stress reduction.         Rehabilitation training in cardiac patients after coronary artery bypass graft (CABG) improves several physiological capacities. The aim of this study was to investigate the effect of eight weeks of combined training on plasma levels and ghrelin gene expression in PBMNC among CABG patients. In this quasi-experimental study, 30 cardiac patients of Javad Al-Aemeh cardiac specialty hospital of Mashhad, who underwent CABG operation, were selected using convenience and purposeful sampling method and randomly divided into two equal groups (n=15): combined training and control. The combined training group performed selected aerobic and resistance training protocols for eight weeks, three sessions per week. Before and after training program, body composition assessments and blood sampling were performed to measure variables. Then, the obtained data were analyzed using independent and dependent t-tests at the significance level of 0.05. Data analysis showed that combined training significantly reduces body weight and BMI and significantly increases plasma ghrelin levels and relative expression of ghrelin gene in PBMNC (P&le;0.05). Additionally, a significant difference was observed between plasma ghrelin levels and relative expression of ghrelin gene in PBMNC of training group compared to control group (P&le;0.05). It seems that eight weeks of combined rehabilitation training increases plasma levels and ghrelin gene expression in PBMNC among CABG patients and can promote desirable physiological adaptations in cardiovascular health.         Coronary heart disease is one of the leading causes of death in today's societies. It is associated to the exercise activity and the expression of some genes involved in the blood cholesterol transmission. Adenosine triphosphate-dependent box protein 8 is a member of ABC transporter family which plays a key role in the metabolism of high-density lipoprotein cholesterol and cholesterol. This study aimed to evaluate the effect of eight weeks of aerobic resistance training on ABCG8 gene expression and lipid profile in CABG patients. 30 men with CABG patients voluntarily selected and randomly divided into the experimental (n = 15) and control (n = 15) groups. The experimental group performed the aerobic resistance training for eight weeks (3 sessions per week) and the control group did not. Blood samples were taken 48 h before the first training session and 48 h after the last training session, while all subjects were fasting. Independent and paired t-test used to analyze the data at a significant level of 0.05. The present study's results showed that the aerobic resistance training group showed a significant increase in the ABCG8 mRNA expression and a significant decrease in LDL-C, triglyceride, and HDL-C as compared to the control group. Due to the findings of present study, it can be concluded that aerobic resistance training by increasing ABCG8 gene expression has a great effect on increasing the cholesterol transfer process; thus, it may improve the CABG patients and prevent cardiovascular diseases.         Health literacy has been considered as the ability of individuals to receive the information and services needed to make appropriate health decisions and as a key outcome in health education. Benefiting from health services, timely receipt of these services and optimal use of health resources, is possible only through health literacy and providing it to the community. The main purpose of this study was to investigate the level of health literacy of patients visiting hospitals in Ahvaz and the influence of social and cultural factors. The research method was descriptive-survey and the statistical population was all patients visiting hospitals in Ahvaz. Cluster sampling method and statistical sampling using sample power software, 170 patients were selected as the sample size. For data collection, Montazeri et al.'s Health Literacy Questionnaire (2014) and Gholizadeh et al.'s Nutrition Culture Questionnaire (2017) were used. The reliability of the questionnaires using Cronbach's alpha was above 0.7 and was confirmed. Data were analyzed by SPSS software using Pearson, Spearman, independent t-test and one-way analysis of variance at the statistical level of 0.05. The results showed that the level of health literacy of patients referred to hospitals in Ahvaz with an average (34.36) is inadequate.         The aim of this study was to investigate the effective factors in coronary artery disease in the families of patients in the fourth peak of prevalence. The statistical population included students of the Islamic Azad University in the electronic unit in the second semester of the academic year 2020-21. Semi-structured interviews were used to collect data. Data collection in March 2021 and data analysis approach was based on Grounded theory (Strauss and Corbin method (1990)). After the interview was completed and saturation was reached, data were collected from 39 participants. Findings showed that factors such as psychological factors, economic, social and psychological factors in the family of patients, affect the fear of coronation. Among the mentioned factors, psychological factors had the first priority (61.14%), social factors had the second priority (29.38%) and economic factors had the third priority (7.11%). In psychological factors, "people's lack of wealth", in social factors, "avoidance of people who sneeze" and in economic factors, the component of "continuous inspection and disinfection of household items" have been the most effective components. Generally; Four important risk factors for coronavirus infection in the family of those infected with the virus are known in the fourth peak, among which it is recommended to pay special attention to psychological factors in these people.         The present study is applied in terms of purpose and in terms of descriptive survey method and combination (qualitative-quantitative). The statistical population of the study includes knowledgeable or knowledgeable people in the field of medical sciences and educational management who were actually involved with the dimensions and subject of the research in the field of hypertension and health literacy. To determine the samples of this section and to determine this group of experts, purposive non-random sampling method was used, in which 23 people were considered as interviewees according to the saturation principle. In a small part, there is a statistical community of medical experts and experts who used non-random and purposeful sampling method and questionnaires were provided to 21 experts in this field. In the qualitative part of the research, the semi-structured interview method was used and in the quantitative part, the matrix questionnaire was used. In order to analyze the data collected in the qualitative section, the three-step method of open coding, axial coding and selective coding has been used. In a small part, the decision of pairwise comparisons has been used. Based on the findings of this study, it can be concluded that the existence of 4 factors of basic literacy with components of reading and writing and knowledge, cultural literacy with components of attitude, values ​​and norms, cognitive literacy with components of application, evaluation, understanding and access, And social literacy with components of communication and interaction can affect the health literacy of hypertensive patients.         Effective usage of coping responses assist individuals in overcoming the issues related to physical, psychological, interpersonal, social, and personal conflicts. Also, psychological integrity is a general alignment that conveys the extent to which the individual is feeling confident, stable, and dynamic in order to see internal and external stimuli in a predictable, transparent, and structured way. The aim of this study is to evaluate the effectiveness of mindfulness-based stress reduction on psychological integrity and coping responses among the patients with asthma. For this purpose, in a quasi experimental study with pre-test, post-test, and control group design, 30 patients with asthma were selected by convenience sampling method and randomly assigned into experimental and control groups (n = 15). The experimental group received eight sessions of mindfulness-based stress reduction treatment (MBSR); whereas, the control group received no intervention. Subjects in both groups were assessed before and after the intervention in order to evaluate their sense of psychological integrity and coping responses. The MANOVA results revealed that MBSR lead to increased sense of psychological integrity, a significant reduction in emotion-focused coping responses, and meaningful increase in problem-focused coping responses. This study showed that MBSR, as a modern intervention, is an effective approach to advance patients&rsquo; sense of psychological integrity and problem-focused coping responses. Manuscript profile

        Objective:This study investigates the environmental factors affecting the treatment process, increasing the recovery quality, and facilitating the patients’ return to the normal life cycle.Background:The intensification of environmental stresses makes the hospital’s environment stressful and non-supportive. Designing a Supportive Therapist Environment (STE) could facilitate the recovery process and improve the patients’ life quality during treatment procedures in inpatient wards. Method: To this goal an exploratory approach was used.The qualitative data were collected by unstructured interviewing six medical specialists and six architecture professors. A questionnaire was distributed among 156 patients to examine the relationship between environmental factors and the possible effect of treatment quality in a surgical ward, Tabriz hospital, Iran.The conditional logistic regression was used to estimate this association. Results: Findings showed a significant relationship between the environmental components and the treatment process quality. That is, the results confirmed positive effects of the architectural interventions on the patients’ sense of calmness, security, and return to life.Conclusion:This study emphasizes on all dimensions including physical, social and semantic, particularly the ceiling as a main factor in controlling stress and maintaining the calmness.STE model can enhance patient satisfaction, and help hospitals create suitable conditions for patients in inpatient wards. To improve the treatment quality, it is necessary to consider mental, semantic, social, and physical components in designing the hospital environments. The study discusses the concepts for understanding the influence of the built environment on human experience, delirium control, treatment process, recovery quality, and return to life cycle.It also introduces further directions for future research.         Abstract Introduction: Psychosomatic diseases are physical diseases that psychological factors are effective in starting or aggravating them, and intensive and short-term dynamic treatment can play an effective role in improving psychological symptoms; Therefore, the present study was conducted with the aim of determining the effectiveness of intensive and short-term dynamic therapy on the difficulty in regulating emotions and attachment styles in psycho-physical patients. Materials and methods: The research method was semi-experimental with a pre-test-post-test design and follow-up with a control group. The research population was all psychosomatic patients referred to psychosomatic clinics in Tehran in 1402. Among them, 30 people were selected by available sampling and randomly assigned to two experimental and control groups (15 people). The experimental group received dynamic therapy 15 sessions of 90 minutes, while the control group did not receive any intervention. Two groups with difficulty in emotion regulation and attachment styles were evaluated in three stages. Two-way analysis of variance with repeated measurements was used to analyze the data. Findings: The average scores of difficulty in emotion regulation and attachment styles before the intervention were 72.25, 58.36, 24.08 in the treatment group and 72.12, 59.16, 23.52 in the control group respectively. (p<0.05), but after the intervention, the average scores of attachment styles in the treatment group increased significantly compared to the control group and showed a decrease in difficulty in emotion regulation (p<0.01). Conclusion: The results of the study indicated the effect of intensive and short-term dynamic therapy intervention on the improvement of psychological symptoms in the experimental group. With caution, it is recommended to use this intervention as a complementary treatment in psycho-physical patients. Manuscript profile
