        This study was conducted to evaluate the health, safety, and the environmental conditions in urban parks, carried out. It have been completed by the FMEA method. After gathering the data from the library and field studies, also as well as reviewing of reports, documents and Journals, the hazards and potential damaging factors and their consequences have were identified. Then, RPN related to each factor was calculated and categorized according to the severity, the degree of probability of detection and occurrence probability. Results of the evaluation showed the largest number of RPN related to equipment in playgrounds as well as the switch structure, switchgear and cable power lines of lighting sources which can lead to many adverse consequences. So these are the priorities that should be omitted or mitigated or controlled by appropriate management strategies. In the end, the results of using the FMEA methodology were compared with the results of periodic evaluation of the health center of Tehran municipality. the results show that the FMEA method can be used as an efficient and modern method to identify and prevention of the occurrence of future events by urban experts, in order to create the ideal urban spaces.         Background: Development and implementation of safety and health policy is one of the necessities of the organization to reduce accidents and occupational diseases. Policy means the decisions and politics and the purpose of implementing the policy are the activities that it performs directly.Objective: Numerous factors lead to the failure to implement occupational safety and health policies. In this study, these factors have been identified and ranked.Methods: This research is applied and strategic in terms of survey. The research population consisted of 15 HSE managers in Kerman province who were selected through non-random and purposeful sampling. Data collection tools are interviews and questionnaires and FMEA technique and integrated approach of AHP and fuzzy TOPSIS have been used to analyze the data.Findings: Based on the findings, important factors influencing the failure to implement safety and health policies are: Lack of proper organizational structure and lack of funding for implementation.Conclusion: According to the research findings, it is necessary that organizations pay more attention to creating and strengthening the structure of safety and health units and monitor the implementation of safety and health policies by estimating and allocating the necessary budget.         Background: Part of the success in implementing policies goes back to the capacity to implement the policy because capacity is the ability to do so.Objective: In this research, the factors leading to the improvement of the capacity to implement policies and the cause and effect relationship between them have been identified.Methods: This research is applied and the research population is15 HSE policy experts in Kerman who have been selected through non-random and purposeful sampling. Data collection tools were interviews and questionnaires and FMEA and fuzzy dimmetel techniques were used to analyze the data.Findings:The index of organizational support in formulating and implementing HSE policies has the highest and the index of creating organizational structure integration in relation to HSE has the least impact on other indicators to improve the capacity to implement these policies.Conclusion:Efforts to improve the capacity to implement HSE policies should be accompanied by more investment to improve the quality of the organization's support index in the development and implementation of HSE policies to strengthen other indicators.         Introduction: The important role of risk and its challenges in organizations is one of the reasons for the importance of examining the risks of production. Considering the classification of the pharmaceutical industry as one of the most important industries, the investment risks in this industry have been analyzed using the Failure mode and effects analysis (FMEA) method. This method is one of the risk management techniques to identify, analyze, and ranking risk factors in order to increase the efficiency of the industry. Methods: This descriptive study suggests FMEA as a quantitative method for identifying production risks and, by computing risk priority numbers (RPNs), represents the probability of their risks and effects in the pharmaceutical industry. Results: In this research, after identifying the risks of drug production through literature review and interviewing experts, a questionnaire was designed that out of 66 identified risks, 19 risks were identified and ranked by experts as important risks. Conclusion: Good manufacturing practices (GMP), exchange rate fluctuation, no marketing strategy and lack of proper estimation of market trends, inventories of raw materials and inventory of supplies was identified as the most important risks of drug production.         Background and Objective: Due to rapid changes in the status of competitors, technology and customer preferences, companies cannot always rely on their existing products. Customers are looking for products with better quality and more advanced and offer new products and companies are forced to respond to the needs, tastes and expectations of our customers, one of the important ways to supply quality products and good performance in product design, failure analysis or risk assessment. Method: this article evaluate the performance of the product components in the computer services industry to the risks and possible consequences in terms of product identification and rank. For risk assessment, data were collected from ten experts of scientific competence and to complete the decision-making matrix. Fuzzy multi-criteria decision-making methods were used for data analysis. These methods in Microsoft Excel were used. Findings: Based on fuzzy VIKOR results, the A5 in the fuzzy approach has the lowest value of Qi and therefore performs better than other options. Based on the results of the fuzzy PROMETHEE, the A4 in the fuzzy risk assessment is larger than φ (a) and therefore performs better than other options. Discussion and Conclusion: Based on fuzzy VIKOR result, A5 is better compared to other options. Based on the results of fuzzy PROMETHEE A4 is better compared to other options.   Manuscript profile

        Background and Objectives: The importance of inhaling the clean air becomes more visible today. In the open air, a number of pollutants are considered as standard pollutants, one of which is particulate matter. The aim of this study was to study the environmental aspects of suspended particles in Tehran Municipality 9th district.Method: In this research, firstly, by referring to the municipality of district 9 and receiving the region information, the activities of the indicator of the producer of suspended particles were identified. Potential damage caused by these activities was identified and given the probability of occurrence, the power of diagnosis and severity of impact on the environment are classified.  Findings: 7 activities were identified and for each activity 17 environmental consequences were identified from the EFMEA method. Of the 17 identified risks, 4 risks include lung disease, cardiovascular disease, digestive diseases and skin diseases that have the highest severity and ranked first to fourth and accounted for 2 risk accidents and driving disorder Traffic signs have the least impact intensity and ranked 16th and 17th ranked TOPSIS. Based on the EFMEA technique, pulmonary diseases with 524 and heart disease with 423 numbers have the highest risk numbers and misdiagnosis of driving tips with 50 and driving accidents with a 52-point risk factor.  Discussion and Conclusion: Regarding the risk assessment in this study, it was revealed that 2 risks of pulmonary disease and heart disease have the greatest impact on the general health of the people in the region and by reducing the environmental risks of the area by taking some control measures.  Manuscript profile

Sadeghieh Terminal as a one of the largest and most strategic metro terminals isactive at the western end of the Tehran Metro Line 2.The aims of this paper is assessing and managingthe environmental risks of Sadeghieh terminal activities with the use of EFMEA method.Material and method: The EFMEA method is a qualitative method that aims to identify andprioritize environmental aspects.Results: The evaluation results shown 46 environmental risks were identified and 8/7 % of risks withlow level, 78/3% of risks with medium level and 13% of risks with high level.Conclusions: The highest risk number is 150 that refer to washing trains activities and sanitary wateruses in Sadeghieh terminal. The proposed corrective action to prevent water waste is collection andtransport wastewater to treatment plant and reuse of produced water.         Implementation of environmental management system, in universities and higher education fields, provides an active and healthy environment towards sustainable development. Implementation of EMS, not only causes continual improvement, but also familiarizes the public with new management system, which be a good pattern for environmental preservation. So, this study was performed to assess the environmental Risks and control and reduce the environmental Risks at the science and research campus of Islamic Azad University. This center is located in a special geographical location, the north Tehran’s fault passes under it, and also construction activities (general plan) to enlarge university and lack of sewage treatment system and unsuitable sewage disposal cause different kinds of environmental impacts. Thus in accordance with the university’s development plans, it was tried to assess the environmental risks to achieve environmental continual improvement. To reach this object the risks were identified and assessed. The assessment of environmental risks has been done through FMEA method. According to risk assessment outcomes, natural hazardous such as earthquake, flood, thunderbolt, explosion caused by constructional process, ground water pollution, natural resource degradation, waste and landscape visual pollution have been identified as high risk activities. Risk levels comparison results show that 63% of the consequences are high risk, 14% are of medium risk and 23% are identified as low risk. Therefore according to the identified risks, the objects, targets, program and indexes of environmental management system were documented and solved environmental management has been suggested. Obviously the main result of research will be the reduction of environmental risks and implementation of EMS in the science and research campus of Islamic Azad University of Tehran. But the effects of these changes and develop this kind of activity due to specific risks arising from changes in prevalent system of nature. Working staff in the agricultural sector of this company at different levels, according to a variety of activities that can be done, are exposed to health and safety risks. This research has been done in order to identify safety, health and environmental aspects and important risks of Salman Farsi Agroindustry Company in 2014. Method: In this study Delphi, FMEA and FMEA & TOPSIS Integrated models are used. Findings: According to Delphi method 57 aspects and risks related to the activities of this industry identified and 13 of them were diagnosed significantly. Risk of exposure to noise with identification code R1 obtain the highest score and transport the heavy parts with identification cod R44 obtain the lowest score respectively. According to the FMEA method, 57 aspects and risks identified based on industry activities and 21 of them became intolerable and clear, as input data to the weighting stage and grading were used combined method TOPSIS and FEMEA. According to this method environmental aspects of waste entering to the environment with identification cod R57 obtained the highest score and falling the centrifuges basket with identification cod R18 obtained the lowest score. Discussion and Conclusion: Establish and implement integrated management systems that can be qualified as a practical proposal affecting elimination or reduction of risk in industrial sector of Salman Farsi agro-industrial Company.         Background and Objective:Risk assessment is a method to determine risk quantitative and qualitative and probable accident consequence on people, materials, equipment and environment. By doing so, existence controlling methods effectiveness are specified and valuable data for decision making in risk mitigating is produced. Method: In this study, GACHSARAN oil refinery production unit is assessed. The purpose of this study is environmental risk assessment of this unit by updating methods and techniques and representing controlling strategies to manage these risks. Simultaneously use of several risk assessment and multi criteria decision making can lead to optimum risk management. To achieve this goal, the Environmental Failure-Mode and Effects Analysis (EFMEA), analytic hierarchy process (AHP) and Technique for Order Preference by Similarity to Ideal Solution (TOPSIS) are used. Findings: At first by site visiting and obtaining expert opinions of the unit, risks are identified and evaluated according to their effects on environmental of five components.         Working in financial institutions and the capital market is one of the most stressful and sensitive jobs in the world. Given the importance of the role of human resources in the performance and events of the capital market, in this study, an attempt has been made to provide a model for analyzing and measuring the critical risks of human resources based on fuzzy FMEA and Pareto principle in the capital market. For this purpose, in the first step, multiple dimensions of risk were identified by semi-structured interviews with HR experts in the capital market, and factors affecting HR risk were identified and fitted to the model by using structural equation modeling method with Smart-PLS software. In the next step, the risk factors of RPN number were determined by FMEA method, which also used the concept of fuzzy logic to solve this deficiency in order to eliminate the weaknesses of FMEA method and to avoid uncertainty. Finally, human resource risks were measured and identified by Pareto Critical Risks based on the fuzzy FMEA method. The results indicate that the critical human resource risks are unethical behaviors (financial fraud), poor leadership, and inadequate rules and policies in the capital market, respectively.         The current research studies and examines the resilience of financial technology businesses. Resilience is determining strategies that can prevent inefficiency and collapse of businesses, or in case of system inefficiency, return to the pre-failure state or better than that in the shortest possible time. In addition, in this research, a comprehensive model of resilience of financial technological businesses and its introduction to the studied society is presented. All the failure modes that cause the inefficiency of financial systems for technological businesses include 12 failure modes that have been identified through research literature and experts' opinions. The statistical population of this research are managers and experts in the field of financial technology business and related groups in Tehran. In the analysis, the FMEA technique is used to determine resilience strategies in financial technology businesses before failure and the new FAAO technique is used to determine resilience strategies of financial technology businesses after failure.         Background and Objectives: Considering that urban and industrial sewage are one of the main factors of pollution entering the environment; environmental effects and technical and operational risks resulting from it have always been considered in the operating phase. But its effects and risks are also important in the construction phase. Therefore, the current research was conducted with the aim of evaluating the environmental and technical risks of the wastewater treatment plant construction project in the west of Ahvaz with the hierarchical analysis process (AHP) in 2021. Material and Methodology: After the study phase, information related to the project was collected through the survey method and presence at the site of the treatment plant, and all executive processes, equipment and facilities were examined. For this purpose, the E-FMEA method and the opinions of experts were used to identify environmental and technical risks. Then, Analytic Hierarchy Process (AHP) was used for weighting and prioritizing the parameters of the used method. Findings: In total, 19 environmental risks and 24 technical risks were identified in the processes of the West Ahvaz treatment plant construction project. Environmental and technical risks were similar in the two stages of construction of buildings and installation of tanks and facilities. In other stages, technical risk was more important than environmental. The number difference between the two technical and environmental risks in the stages of excavation, installation of foundation and skeleton of structures was 50, 25 and 66%, respectively. Discussion and Conclusion: The comparison of all risks showed that technical risks were about 26% more than environmental risks. In general, the results showed that most of the environmental risks were at an acceptable level and 8 technical risks needed to be resolved as soon as possible. Manuscript profile

        This study was conducted with the aim of environmental risk assessment of Shohadaye Khalij Fars Agro-industrial Complex situated in kilometer 75 of Ahvaz - Khorramshahr Road. In this paper, after reviewing activities, measuring contamination parameters and processes of sugar production salon of the Complex, environmental risk assessment was performed via TOPMEFEA. This is a hybrid method for the environmental risk assessment which is composed of a combination of the two methods of TOPSIS and EFMEA. For this purpose, in accordance with the method of analysis of failure modes and its impacts on the environment, some worksheets were first designed to assess the environmental risks. In these worksheets, variables such as the identification process of potential failure modes, potential effects of failures, potential causes of failure, initial assessment of the environmental aspects, were studied as environmental aspects. Then, using brainstorming method environmental impacts of each activity were identified and evaluated by relevant personnel and experts. The results showed the entry of contaminants and chemicals to aquatic environment due to sugar production process with the risk priority number of 16 with first priority, and the entry of chemical compounds and carbohydrates into the aquatic environment due to syrup refining, and contaminates. Considering that drainage in this Complex will have the most significant outcomes, the establishment of an industrial refinery and the application of advanced methods of wastewater treatment were among the proposals to reduce the most important risk factors.         The petrochemical industry is one of the most important industries in the oil and gas sector because of the enormous volume of facilities and processes, which have unavoidable risks and risks. Different risk assessment methods are used to manage these risks, especially in Engineering Procurement Construction (EPC) projects. On the other hand, considering the number and variety of risks in Engineering Procurement Construction (EPC) projects, determining their weight and ranking is of particular importance. This research is applied in purpose and is conducted in the head office of one of the petrochemical companies and two groups consist of experts of contract education and contracting unit (project management contracts) to collect the selected information that answered the questionnaires. In this study, the identified risks were evaluated and rated using the failure modes and effects analysis (FMEA) technique, and the critical risks were weighted and ranked using Analytical Network Process (ANP) and superconducting software. According to the results of the study, contract estimation risks were the least important and engineering sector risks were the most important. The results also showed that the risks of logistics sector, risks in contract estimation (values and costs), financial sector risks and finally engineering (design) risks are more important respectively. In addition to assessing the risk of ERP establishment, the purpose of the present study is to compare the risk ratings in both pre-establishment and establishment phases. Moreover, the heuristic AHP method considered during the establishment phase combined with FMEA provides more accurate results.         In paying attention to the recent years, domestic industries encounter severe slump and some of them has become insolvent, some are also at the beginning of bankruptcy. The existence of a research that would be able to applicability and simply show the causes of domestic industries bankruptcy, and make it flexible to the government`s honorable attendants, congress representatives, judicature, and other governmental and private organizations, has been less seen. Therefore, the writers of this article have tried at the first step to extract the main causes of bankruptcy and slump domestic industries from the writing and speeches of country`s attendants and experts and specialists. At the second step, the causes are presented to specialists and advisers of Ministry of Industry and Mining. Then by using Quality Function Deployment (QFD) and Failure Mode and Effect Analysis (FMEA) more than 90 problems are extracted, classified and prioritized, and then the obtained results are used as input to draw cause and effect diagram (fishbone diagram). In this article to draw cause and effect diagram, the bankruptcy causes and slump domestic industries are categorized into 8 parts: 1. government, 2. domestic industries, 3. goods smuggling, 4. bank, 5. judicature, 6. Congress, 7. environment, and 8. people. On the one hand, this diagram can simply show the domestic industries problems based on their importance and on the other hand specify the related organizations to solve the problems. This current research from the viewpoint of purpose is an active research and from the aspect of approach and nature is a descriptive-applicable research in which the library approach is used to gather information, and also experts judgment are used for validity and reliability.         Due to the growing trend of use of the outsourcing approach in organizations, Gas Company of Chaharmahal and Bakhtiari province, keep pace with developments in the outsourcing of Iran, implementing the most of their capital projects as the treaty. But according to the tenure of parts of operation service is still undertaking company and planning for outsourcing during the coming years in the agenda is located, in order to correctly use of outsourcing approach and improvement of Performance Company’s project, correct evaluation of risks before the outsourcing seems necessary in this organization. The purpose of this study provided a useful framework for identifying risks in the company's outsourcing projects and prioritizing them uses FMEA technique. Therefore, investigating the studies conducted, major risks and their effective factors were identified and classified considering the parameters of quality, cost and delivery time. Then, using of FMEA technique and based on the priority level of acquired risk severity, occurrence and detection, attempting to quantify and prioritize identified factors and providing corrective actions to control and reduce the risks. In this study, using a questionnaire survey and interviews with seven of the top executives of companies that are directly related to the issue is performed. Based on acquiring results, structural defects, functional defects, and delay in outsourcing project's implementation are more significant and currency exchange fluctuations factors, uncertainty of changing technology and customer requirements, lack of experience, expertise and knowledge in the field of outsourcing and lack of required skills for performance projects in high level and requires greater managerial control.   Manuscript profile

        Health and safety policies are implemented in the organizations to reduce occupational accidents and diseases and prevent waste of resources. Policy means the decisions and implementing the policy means the activities that help implementation directly. Numerous measures will reduce the failure to implement occupational safety and health policies. In this research, these factors are identified and ranked.This research is applied and adopts survey as its strategy. The research population consists of 15 HSE managers in Kerman province who are selected through non-random and purposive sampling. Data collection tools are interviews and questionnaires and FMEA technique and integrated approach of AHP and fuzzy TOPSIS are used to analyze the data. Based on the findings, important and effective measures to reduce the failure of safety and health policies implementationinclude reducing the change of managers and requiring new managers to implement safety and health policies, developing scientific programs in accordance with the organization's capabilities and creating an appropriate organizational structure. According to the research findings, it is necessary for organizations to pay more attention to creating and strengthening the structure of safety and health units by carefully planning and changing the approach of managers.         Six-sigma methodology is known as a comprehensive and quantitative method for implementing improvement in the organizations. In this paper, attempts are made to implement this method at SANAM Company. To this end, first, we will consider the process of manufacturing plastic products based on the costumer perspective. Then, we will explain six-sigma methodology and DMAIC cycle. Finally, the problems and failures in the production line as elicited through brain storming meetings and using FMEA and Pareto method through MINITAB software are reported. Then, we will choose the related aspects and find the current sigma level of the company. Finally, suggestions are made to improve the production process based on the selected aspects and the new sigma level of the company will be calculated and we will see that the sigma level and the company’s profit was increased saliently.         The failure mode and effects analysis (FMEA) is a qualitative, Inductive and effective method for detecting errors, faults, and failures in a system and fuzzy logic can improve that technique with more logical outputs. Moreover, the fault tree analysis (FTA) as a probabilistic risk assessment method is among the effective technique for calculating the probability of errors, faults, failures, reliability and safety integrity level (SIL) verification resulting in certain events at higher levels. The FTA also detects the main causes of events in complicated systems. Although this technique appears to be time-consuming in systems with many diverse components, it is considered a powerful tool. In this paper, the fuzzy FMEA analyzes the failure modes in a hypothetical system. After that, the process with the highest risk is selected as the input of an FTA.         The purpose of this paper was to Environmental risks Assessment of the Construction Project using the EFMEA Method with Emphasis on the risks affecting water pollution. In this study, through field visits to the construction project, the project activities were fully identified and then the environmental risks were identified. In order to estimate the amount of risks, according to experts, checks and tables, the EFMEA method was used. Following the work process, solutions and control measures were presented and the risk number was recalculated. According to the results of this study, the highest number of risks was related to secondary accidents (250) and the lowest number of risks was related to manual freight transport activity (8). After presenting control measures, it was observed that the numerical value of environmental risks, especially risks affecting water pollution has decreased. Manuscript profile

        This study was conducted with the goal of investigating the amount of Chrome heavy metal in Firouz Abad river of Tehran. For this purpose, sampling from water was conducted by determining 5 measurement stations in the river path in the spring, summer and autumn of 2007. Analysis of samples was performed using Atomic Absorption method. The results of this study show that the levels of chrome metal measured in the third, fourth and fifth stations with the averages of 0.503mg/l, 1.29mg/l and 2.62mg/l, respectively are above the allowed level of chrome in the run-off determined by Environment Preservation Organization (0.5mg/l). In order to estimate the level of environmental risks of chrome in the environment of the affected area, the Environmental Failure Modes and Effects Analysis and its effects on environment was used. For this purpose, a table was first designed based on each station in which chrome concentration was graded within the scores span of 1 to 50 from the respects of risks severity and possible consequences. Then, Risk Priority Number (RPN) was calculated for each station from the product of risks severity in possible consequence. In the stations in which the level of chrome heavy metal was above the standard level, some measures were suggested for minimizing RPN. In this study, the level of environmental risks of this metal was estimated in 5 stations based on RPN as follows: RPN of first station = 2 < RPN of second station = 10 < RPN of thied station = 30 < RPN of forth station = 200 < RPN of fifth station = 450 Due to irrigation of agricultural lands by this river especially vegetable farming and growing summer crops in south of Tehran, the excess concentration of chrome has increased significant absorption of the aforesaid heavy metal in plants which its consequence is the entrance of this metal to food chain and its bad effects on human.         Synthetic man-made Persian Gulf Martyrs Lake in the North-West of Tehran, in the 22nd region, with a volume of 10 million cubic meters, an area of 224 hectares and an average depth of 10 meters, is the largest man-made lake in Iran. In this study to assess the risks arising from the exploitation phase of Persian Gulf Martyrs Lake, multi-criteria decision making, MADM, was used. In this regard, the risks of exploitation using questionnaire, were also analyzed using SPSS software and TOPSIS method. For the risk factors and prioritization of the risks of the lake, Analytical Hierarchy Process (AHP) was used. In this method, for the hierarchical structure analysis of the risks of the lake, matrix pair comparison was performed using pairwise comparison of each of the criteria options and in relation to each other. Finally, to determine the probability of occurrence of each risk, Expert Choice software was used; also using FMEA logic, the results were assessed. Based on the results, it was found that in the physical & chemical environment, impact on ground water and sedimentation was the most important risk factor and eutrophication was identified as the most important biological risk.         Nowadays, as projects become more complex, uncertainty in the implementation of projects also increases. Project risk includes threats to project goals. Risk management is one of the main parts of strategic management of any project and includes processes through which risks related to activities can be identified methodically. The main purpose of this study is to identify and analyze risk, using the integrated approach of PMBOK and RFMEA methods in petrochemical industry projects. The statistical population consists of employer, contract management, engineering consultant, and contractors of the Kermanshah petrochemical industries company. Of whom 10 were selected for the qualitative part and 248 were selected for the quantitative part (regression analysis). Questionnaires were handed to the selected population. 220 of which, were filled out and returned acceptably. As a result, a total of 22 risks were identified as critical risks (with RPN of 1) and the role of management, organizational, and environmental factors in risk management was investigated.
