        Introduction: The importance of the referral system in the health system is very high so that it is stated that 80 to 90% of the first-level health care clients are diagnosable and treatable. Therefore, Referral system in the health system is very important so that it is mentioned that 80 to 90 percent of patients in the first level of health cares are recognizable and treatable. Therefore, modification of referral system can reduce greatly amount of specialist and supersonic referral of hospitals. The purpose of this paper is to investigate on the Effect of Health Policy and structure of health insurance on referral system in the Urban Family Physician Program in Iran in order to improve and develop the first, second, and third level services of the referral system in Iran. Methods: The present study is a descriptive-correlation research. The policy makings conducted, as well as the status of the insurance system of different countries was extracted through systematic identification of the Urban Family Physician's referral system. Then the effects of these factors on the referral system were identified using a researcher-made questionnaire and statistical analysis of factor analysis and T test. Statistical population was included 400 subjects who were expert on family physicians at the national level. The Data Sufficiency was evaluated by the Kaiser-Meyer-Olkin’s (KMO) test. Reliability of test was calculated and confirmed according to Cronbach's Alpha and Combined Reliability (CR). Also, the validity of the test was calculated and confirmed according to the average variance extracted (AVE). Results: The effect of proper health insurance structure and transparent financial rules on the referral system of the urban family physician and proper health system’s policy makings were proved through coefficients of 0.860 and 0.804, respectively. In the health policy factor, the most important variable is the question number 9 on supervision of the provision of services at first, second, and third level of referring in which factor loading is 0.774.also, in the insurance structure dimension , the most important variable is the question number 12 on applying restrictions in order to refer patients in the pharmacy clinic having in which factor loading is 0.793. Conclusion: It can be calculated that proper health insurance structure and proper health system’s policy play important roles for success and improving referral system of urban family physician.         Introduction: The philosophical dimensions of lean management in health section, and especially in hospitals which could renew their function and contribute to their development in providing improved services are of particular importance.The present study aims to investigate the effect of waste control, lean techniques and visual systems on lean management in hospitals of Shahid Beheshti University of Medical Sciences (SBMU). Methods: Considering the results, the present case study falls in the applied category. The research populations were managers at different levels of senior, middle, and operational in the SBMU teaching hospitals. The data were obtained through researcher-made questionnaire that its validity confirmed by a group of elite scientific and executive experts in the field of lean management and its reliability calculated 82 percent using Cronbach's alpha. Results: The obtained findings revealed that all the three factors of waste control, lean techniques, and visual systems significantly loaded on the lean management. The results of the conducted factor analysis on the basis of goodness of fit indices NFI=95%, GFI=92%, and RMSEA=0.052% verified fitness of the model. Conclusion: Considering the effect of waste control, lean techniques, and visual systems on lean management in the teaching hospitals, examining hospital infrastructures and the use of self-assessment system for feasibility of lean execution could be effective in establishment of lean management in hospitals. The purpose of this study was to determine the factors related to financing in pre-hospital and hospital emergency rooms in Iran in order to improve the provision of services to community members. Methods: The present research is descriptive and comparative. Developed countries in different parts of the world based on the similarities of health systems based on geographical area, the level of development of health and medical indicators, payment system and financing method were selected. The data on the financing process in pre-hospital and hospital emergency rooms of the countries under study were collected and then, based on the adaptive tables, effective factors on the financing of pre-hospital and hospital emergencies in the countries studied in the areas of financial resources management, budget, payment system Service providers and insurance coverage were extracted. Results: Findings indicate that in many countries, lack of funding in pre-hospital and hospital emergency rooms is an important barrier to providing services in this area, and this may be due to several factors: lack of adequate funding, inadequate funding, The pay system is not transparent to service providers and the inability to manage financial resources. Emergency financing is well defined, there is a lot of distance. Key and backup processes are not properly designed to fund, and there is no specific framework for identifying and deploying financing processes to implement policy and strategy in pre-hospital and hospital emergency situations. Conclusion: The need to reform the emergency services by reorganizing the emergency and improving the emergency by applying effective approaches to improving the financing of emergency funds in the form of an integrated model and in the form of a change in the type of look at planning and financial management in the emergency department with a view to the viability of services that With a clear indication of the partnership-driven, community-based, citizen-oriented, converging and synergistic approach to different types of emergency management, social capital and appropriate insurance coverage for emergency services.
