the managed care is an example of the new system of care taking which is designed to control the careexpense, manage the utilization of the services, and evaluate the services done for the improvement ofthe quality and make a proper cost- effectiveness through the managed care organization .the mostimportant objective is, study of planning, organization and control in selected country and designing amodel for Iran.it is a practical research and trough a comparative – descriptive procedure like those performed in theUSA, Australia, UK, Malaysia. The utilization of the descriptive statistics for the analysis of thequestionnaire by means of Delphi technique is done.in order to encourage the care taking of the patients in a proper way and to increase the economicaleffectiveness of the system in the investigated countries , some programs like" Integrated Care" are putin to practice, so that the expenses are controlled to a large extent. Primary care groups in UK are thesimplest form and managed care in the USA is the most complex process the integrated care. Planning, organizing and control of this model is accepted by mean 78.2 ± 1.15, 70.14 ± 1.6 and78.76 ± 3.6(Pvalu .05).it seems that, health care expenditure could be reduced considerably through establishing managedcare organizations (MCOs). So، this model can create a new way of insurance and health careservices. Manuscript profile

In home health care (HHC) problem, the assignment of medical teams to patients and scheduling medical members are done manually, which is a time-consuming process and sometimes are not optimized. In this article, we examine the scheduling assignment of medical teams to the patients with a new innovative approach based on resource constrained project scheduling problem. Advantages of proposed approach can be included the use of theorems, standard benchmark problems, and various (meta-innovative) scheduling methods to improve assignment quality, as well as determining the minimum number of human resources required to cover all medical services requested by patients. In this approach, we solve the HHC problem by defining a dynamic priority rule and using the parallel scheduling scheme. Evaluation criteria for evaluating the quality of feasible solutions resulting from scheduling include minimizing the travel time of medical team members, reducing overtimes, maximizing the number of human resources which are used, and so on. The set of feasible solutions satisfy to some constraints such as the total working time in the contracts, the hardware time window of each job, the mandatory breaking time, and the optimal assignment of the medical team to patients (according to the type of service requested by the patient). The numerical results obtained from calling the proposed algorithm on the Kolisch’s benchmarks are presented to evaluate the performance of proposed approach. Based on the results, the proposed algorithm has a high ability to solve the HHC problem. Manuscript profile

Recently with the growing population and the consequences of factors such as the increase in the number of elderly and patients with chronic diseases, the demand for receiving Home Health Care (HHC) is increasing. HHC services providers are looking for the optimal solutions in planning and scheduling HHC service delivery to maximize the satisfaction of patients and nurses in addition to minimizing the costs to patients. Accordingly on the one hand, the patients prefer to be visited at some specific periods based on their nursing skills. On the other hand, nurses are willing to provide services during their desired time windows. Following the rules corresponding to the working times in the contract, observing the soft and hard time windows, and taking required breaks are some of the restrictions that must be considered. The main objectives of this paper are to minimize the total traveling times and overtime of all nurses and to maximize the satisfaction level of patients as well as nurses, which are achieved through a multi-objective mathematical programming model. The proposed model considers the preferences of the nurses, as well as their patience. Moreover it establishes mandatory breaks for nurses after a certain period of work to assign qualified nurses to patients, optimize schedules, and determine the route of nurses, and provides high-quality services. Finally by applying the proposed model to a set of different random test problems, and by considering the stopping criterion on the problem solving time, we analyze the numerical results corresponding to optimal scheduling and allocation. Introduction: The impact of different effective factors on health expenditure appears in the public health and health promotion. Health is a key element in economic, social, and political development and has an important role in different infrastructure of society. Considering the previous studies in this field with the classic assumption and according to the fact that existence of spatial dependence between the observations and disregarding these effects, the method of estimating conventional econometrics will be biased. Therefore, the main aim of this study is to consider the spatial dependence between observations. Methods: we use spatial econometrics model and spatial Durbin model in order to analyze the effective variables on health expenditure in 79 selected developing countries during (1995-2014). Results: The empirical results have indicated that there is a significant negative relationship between misery index and health expenditure. It means that if the weighted average of the logarithms of misery index increases 1 percent in neighboring countries, the average logarithm of health expenditures will decrease 0.13 percent in targeted country. Moreover, the effects of per capita income (0.54) and life expectancy (0.49) on per capita health care expenditure are significantly positive. Conclusion: The results of this study indicate that if governments do not have appropriate policies to prevent the illegal immigration of foreign nationals and face difficulties in employing labor in their country, the health of the community will be compromised in long term because of the escalation of unemployment and Lack of inhibition of inflation. Introduction: The jobs that employees have the role of care are naturally susceptible to various types of physical and mental illnesses. Psychological well-being is one of the variables that can act as deterrent and moderating against such issues and problems. This study aims to investigate the role of psychological empowerment in the prediction of psychological well - being in health care workers. Methods: This research is descriptive – correlative study. The statistical population of this study was 720 people of the health care workers of the city of Qayen, among which, using cluster random sampling method, and 250 people were selected as sample. Data were gathered using psychological empowerment questionnaire of Spritzer and Mishra (1995) and psychological well-being of Riff (1995). The results were analyzed using SPSS software whit Pearson correlation coefficient and regression test. Results: The results showed a positive and significant relationship between three dimensions of psychological empowerment (autonomy, meaningfulness, and trust) with psychological well-being, but there was not relationship between two other dimensions of psychological empowerment (competence and effectiveness) with psychological well-being.The results of the regression analysis showed that, among the dimensions of psychological empowerment, two dimensions (trust and self – determination) have the power to predict psychological well - being. Conclusion: Therefore, human resource needs and priorities must always be considered and as far as possible, the empowerment programs in the organization must be carried out. In health care provider organizations (HCPOs), there are several sub-units, simultaneously providing health and preventive services to the population covered, in which measuring productivity is one of the most important challenges for the managers of these organizations. Hence, the purpose of this study is to provide an integrative approach of data envelopment analysis (DEA) and Malmquist productivity index (MPI) to monitor the productivity in Iranian Petroleum Industry Health Organization (PIHO) as an HCPO. In this study, using the indicators related to efficiency and defining the specialized indicators affecting the performance of the sub-units of this organization, the growth rate of the performance of the sub-units was determined through calculating four distance functions and Malmquist productivity Index. According to the results of this study, 27 specialized indicators in the fields of direct and indirect health and preventive services for monitoring the effectiveness of proposed HCPO have been presented. Also, the growth rate of productivity of the fifteen areas of the organization under study has been evaluated from 2019 to 2021. The innovative aspect of this article lies in the definition of effectiveness measurement indicators and presentation of a theoretical framework for monitoring the rate of productivity in HCPOs. The findings of this applied research in health service organizations can be used to enhance capacity in different areas of health care and save resources. Manuscript profile
