        Background:Foresight practically; relying on the interaction between a wide network of stakeholders and experts; is pursue shaping a favorable image of the future. The core element of this paper focuses on:What are the most prominent factors which play a fundamental role in foresight projects successful functioning. Objects: Identifying:(1)the critical elements in Successful foresight and,(2)prominent pitfalls and challenges in the foresight process. Methodology: The first step:Through investigating of library resources,(80)causes identified as effective factors in the foresight process. The second step: These factors in order to assess the face and content validity were presented to (10)experts in the field of foresight. The next:A questionnaire; with a fuzzy cognitive map approach; inclusive of (30)essential factors was designed and between(8) experts distributed. Ultimately:By using FCMAPPER and Excell Software Data analyzed. Findings:In terms of input index (Influence of other factors) three concepts;"using appropriate methodology and method of foresight", "realistic making decisions" and "implementing policies and actions"respectively are most influential factors. In terms of output index (effective of other factor) three concepts:"using appropriate methodology and method of foresight","the clarity of the rational principles and the logical reasons for performing foresight for those involved in foresight "and "most prominent futurists participate in the foresight project"sequentially are most effective factors. Conclusion:The most fundamental factor in more realistic making decisions and proposed policies and actions in consequence of implementing foresight project depends on the deepening and updating of knowledge of the country's future prospects in the field of familiarization with the latest methodology and modern methods of foresight.         Introduction: Despite the importance of the service sector and its contribution to the world economy, studies on the supply chain of the service sector in comparison with the industrial sector are very rare The purpose of this paper are: identify effective concepts on the sustainable supply chain of hospitals, and design a sustainable supply chain model for the hospital.  Methods: This study is an applied descriptive survey. its nature is a type of qualitative and Delphi methods. At first, using deep interviews, effective concepts on the sustainable supply chain of the hospital were extracted. Then, using the Delphi method, the concepts were placed in the form of 15 main concepts. then, using of Type-2 fuzzy cognitive mapping method a causal  model of sustainable supply chain were desined. Results: Effective concepts on the sustainable supply chain include 68 concepts that are in the form of the 15 main concepts such as: demand management, resource  and capacity management, customer relationship management, supplier relationship management, service management, information management, financial performance management, Attention to the environment, contamination, energy consumption, legal requirements, employees, community and stakeholders, social accountability and business ethics. Based 15 concepts, the mapping of the sustainable supply chain of the hospital was designed. The results indicate that service delivery management is highly central among other concepts.  Conclusion: By focusing on concepts such as service management and capacity and resources management that have a high level of focus, a sustained supply chain supply chain can be improved.         Nowadays, due to the population’s growth and the expansion of science and new technologies, various businesses have been created in society. These businesses need to pay special attention to human resources in order to have good productivity and not be weak in front of their competitors. This research aims to design a model to determine the factors affecting the emergance of human resource black holes using a fuzzy cognitive map (FCM). The statistical population of this research includes experts familiar with the subject at Lorestan University and Azad University, Lorestan branch whose sample members were selected using the snowball sampling method based on the principle of theoretical saturation. The data collection instrument consisted of the interviews in the qualitative part, whose validity was confirwmed using content validity, and their reliability was calculated using intra-coder inter-coder reliability with a coefficient of 78. The data collection instrument is the quantitative part included a questionnaire, whose validity and reliability were confirmed by content validity and test- retest reliability, whose coefficient was found to be 84. In this research, the content analysis and coding methods were used in the qualitative part and the FCM was used in the quantitative part to analyze the data. The findings in the qualitative part included identifying factors affecting the emergeance of human resource black holes, and the findings of the quantitative part included the ranking of these factors based on the degree of importance. The results of the research showed that a preference for engagement over expertise with the highest capacity for influence and consequently the highest degree of centrality was identified as the most important factor influencing the emergence of human resource black holes. Following this, the supervisors' inattention to the organization's problems and events, lack of proper performance evaluation system, individualism, and managers' inefficiency were identified as other important factors affecting the emergence of human resources black holes.
