Comparison of the effect of resistance, endurance and concurrent exercises on adiponectin gene expression in hand and foot muscle tissues of male Wistar rats

Abstract :

Background & Aim: Adiponectin is a type of adipokine that is involved in sugar and lipid metabolism, this adipokine is in addition to the tissue that it is like and can become. The present study was designed to compare the effect of endurance, resistance and combined exercises on the level of adiponectin gene expression in the hand and leg muscles of Wistar rats. Materials & methods: The current study is an experimental research in which 40 healthy male Wistar rats were divided into four ten groups of control, resistance training, endurance and combined. Each group did their own exercises for 8 weeks. Finally, hand and foot muscles were examined to measure the adiponectin gene. Results: The results of the analyzes showed that the level of adiponectin gene expression was different among all groups (P < 0.001). The relative expression of adiponectin gene in all training groups increased significantly compared to the control group (P < 0.001).Also, comparing exercise groups together, combined exercises had the greatest effect and resistance exercise had the least effect. In intra-group comparison, adiponectin gene expression was higher in hand muscle than leg muscle during resistance training (P = 0.047), but gene expression was higher in leg muscle during endurance training (P = 0.029). These two muscles were not significantly different during combined training (P = 0.086). Conclusion: According to the results of this study, all types of sports training can increase the expression of adiponectin gene, but combined, endurance and resistance training have the greatest effect, respectively.

بررسی اثر پروتئین فعال کننده نوتروفیلی هلیکوباکتر پیلوری بر میزان لکوترینهایC4, B4  و التهاب مجاری هوایی در موش مدل آسم آلرژیک

چکیده

زمینه و هدف: پروتئین فعال کننده نوتروفیلی هلیکوباکتر پیلوری (HP-NAP)  به عنوان یک ایمونوژن برای برانگیختن پاسخ های آنتی ژنی یا یک تعدیل کننده ایمنی برای تنظیم ایمنی سلول های T عمل می کند. این پروتئین می تواند یک پاسخ Th2 منجر به پاسخ آلرژیک را به سمت پاسخ Th1 سوق دهد .همچنین احتمال می رود بر میزان تولید لکوترینهای C4, B4 و نهایتا بیماری آسم نیز تاثیر بگذارد. هدف از این مطالعه بررسی تاثیر HP-NAP بر میزان لکوترینهای C4, B4 در موش های مدل آسم آلرژیک و مقایسه آن با تاثیرات خود هلیکو باکترپیلوری (Helicobacter pylori) می باشد.

مواد وروشها: در ابتدا سویه استاندارد هلیکوباکتر پیلوری و پپتید HP-NAP طبق مطالعه قبلی تهیه شد. سپس 40 سر موش نر بالغ Blab c   6 تا 8 هفته‌ای برای مدل‌سازی و ایجاد آسم آلرژیک تهیه و به 4 گروه 10 تایی تقسیم و برای مطالعه تغییرات لکوترینهای مذکورانتخاب شدند.

نتایج: در اندازه گیری سطح لکوترینهای C4, B4 ،با استفاده از روش الایزا مشاهده شد که سطح C4, B4 گروه آسمی به طور معناداری نسبت به گروه سالم افزایش دارد. در حالی که تیمارها با هلیکوباکتر و HP-NAP منجر به کاهش معنی‌داری در سطوح C4, B4 نسبت به گروه آسمی نشد.

نتیجه گیری: با توجه به اثرات قوی تر HP-NAP نسبت به خود هلیکو باکتر در کاهش سطح لکوترینهای C4, B4 ، HP-NAP توانست در حیوانات آسمی میزان لکوترینهای C4, B4را کاهش دهد ولی میزان کاهش آن نسبت به گروه آسمی معنی دار نبود .بنابراین مطالعات بیشتر پیشنهاد می گردد.

مقدمه

آسم آلرژیک یک بیماری مزمن التهابی برونش ها ی ریه است که در پاسخ به آلرژنها و با ترشح بیش از حد موکوس در اپیتلیوم برونش ها و ضخیم شدن دیواره آنها و متاپلاژیای سلولهای جامی شکل برونش همراه است. سیستم ایمنی در مقابل بیماریزایی آسم بسیار پیچیده عمل می کند. بیشتر مطالعات بر روی گیرنده هایی مانند سلول های Th1,Th2، Th17، (T-regs، و سلول های دندریتیک (DCs) انجام شده، که نشان می دهد این سلولها با هم تشکیل یک شبکه پیچیده بین سلولی را می دهند که این خود باعث ایجاد دیدگاه وسیعی برای تحقیقات ایمونولوژیکی آسم شده است (3-1). پیشگیری، شناخت، کنترل و درمان آسم برای همه جمعیت ها ضروری است و حمله آسم نتیجه اجتناب ناپذیر عملکرد نامناسب سیستم ایمنی است. در ارزیابی هیستوپاتولوژیک، تجمع و نفوذ ائوزینوفیل ها نشان دهنده التهاب برونش است. (6-4). مشاهده شده که متابولیتهای میکروبی می توانند مستقیماً روی سلول های سیستم ایمنی ذاتی از جمله سلول های NK و DCs تأثیر گذاشته و ترشح اینترفرون IFN-γ، IL-12 و IFN-α را تحریک نمایند و باعث سوییچ عملکرد سلول های Th2 به سلولهای Th1 شوند. البته این فرآیندها هنوز نیاز به مطالعه بیشتری دارند (8-7). بنابراین، با تجویز باکتری ها یا اجزای آنها شاید بتوان از ایجاد آسم جلوگیری کرد (9). جالب توجه است، ارتباط بین عفونت هلیکوباکتر پیلوری و کاهش خطر آسم آلرژیک مشاهده شده است (11-10). هلیکوباکتر پیلوری یک باکتری مارپیچی میکرو آئروفیل گرم منفی است که می تواند باعث ایجاد انواع بیماری ها مانند التهاب مزمن، زخم معده و سرطان معده شود. (7، 14-12). پروتئین فعال کننده نوتروفیل هلیکوباکتر پیلوری (HP-NAP)، یک پروتئین فعال کننده گیرنده TLR2 است که می تواند به شدت نوتروفیل ها، مونوسیت ها و DCها را تحریک نماید(16-15)HP-NAP . به عنوان یک ایمونومدولاتور می تواند به صورت داخل صفاقی یا داخل بینی تجویز شود (17).

از طرفی مشاهده شده که لوکوترین ها (LTs) نقش مهمی را در پاتوفیزیولوژی آسم ایفا می کنند (22-18) از جمله لیپیدهای التهابی موسوم به سیستئینیل لکوترین LTC4) ، LTD4، LTE4 )و LTB4، که واسطه های لیپیدی بیولوژیکی قوی می باشند. (23-19). لوکوترین ها می توانند پاکسازی موکوسیلیاری را مختل و ترشح موکوس را افزایش و لکوسیت ها را به طور کموتاکتیکی به مجاری هوایی جذب کنند و همچنین نفوذپذیری عروق ریوی را برای ایجاد ادم تسهیل کنند (22-21). لکوترین های C4 و D4 استنشاقی 1000 برابر قوی تر از هیستامین در ایجاد انسداد جریان هوا در افراد عادی هستند و مدت اثر طولانی تری دارند (23). در بیماران مبتلا به آسم، راه های هوایی 100 تا 1000 برابر بیشتر از افراد عادی به لکوترین های D4 و E4 استنشاقی حساس هستند (23). لکوترین‌های استنشاقی C4 و D4 همچنین حساسیت بیش از حد برونش را به عوامل دارویی مانند متاکولین یا هیستامین افزایش می‌دهند (24). این پاسخ‌ها به لکوترین‌های اگزوژن با ویژگی‌های بالینی انسداد راه هوایی در آسم همراه است و نقش بیولوژیکی این ترکیبات را در این بیماری نشان می‌دهد. علاوه بر این، لوکوترین ها در پلاسما، ادرار، ترشحات بینی، خلط و مایع شستشوی برونکوآلوئولار  (BALF) از بیماران مبتلا به شروع خود به خودی آسم پس از مواجهه با آنتی ژن، شناسایی شده اند (25).

بنابراین با توجه به تاثیرات LTها در پایداری و تشدید بیماری آسم و از آنجاییکه هیچ مطالعه دیگری درباره اثرات  HP-NAPبر روی میزان LT نشده است، در این پژوهش تاثیرات HP-NAP بر میزان LTها ونهایتاً پیدا کردن راه کاری برای کنترل و احیاناً درمان بیماری آسم، برررسی شد.

مواد و روش ها

در ابتدا سویه استاندارد هلیکوباکتر پیلوری و پپتید HP-NAP تهیه شد. سپس تعداد 40 سر موش نر بالغ 6 تا 8 هفته ای برای مدل سازی و ایجاد حالت آسم آلرژیک برای مطالعه تغییرات لکوترینها تهیه شد. به این ترتیب که در ابتدا موش ها در 4 گروه 10 تایی انتخاب و سپس در گروه های مدل تحت حساس سازی و چلنج توسط پروتئین اوآلبومین Ovalbumin (OVA) از طریق داخل صفاق و نایی قرار داده شد تا علائم آسم در این حیوانات ظاهر شود. بدین صورت که ابتدا در روز 1 و 14 OVA به موشهای مورد نظر طبق گروه بندی انجام شده از طریق داخل صفاقی تزریق سپس در روزهای 24، 26، 28 و 30 از طریق درون نایی و با استفاده از دستگاه نبولایزر با OVA حساس سازی انجام گرفت (26). همچنین طبق گروه بندی در روزهای 25، 27 و 29 موشها تحت تیمار با  H. pylori و HP-NAP از طریق درون نایی و با استفاده از دستگاه نبولایزر قرار گرفتند. در کل گروه های مورد مطالعه شامل موارد ذیل هستند:

1) موش حساس شده با OVA (کنترل مثبت)، 2) موش سالم که فقط PBS دریافت می کنند و OVA دریافت نکرده و حساس نشده است (کنترل منفی)، 3) موش حساس شده با OVA و دریافت کننده هلیکوباکتر، 4) موش حساس شده با OVA و دریافت کننده HP-NAP. درنهایت در روز 31 عمل یوتانزی روی موشها انجام و جهت انجام مراحل ذیل در نظر گرفته شد.

برای مطالعه تغییرات لکوترینهای C4, B4 ابتدا مایع BALF استخراج شد. برای استخراج مایع BALF ابتدا یک دهم میلی لیتر PBS بوسیله لوله قابل انعطاف از طریق نای وارد ریه حیوان شد و سپس استخراج شد. برای سنجش فاکتورهای مورد نظر با استفاده از روش الایزا ، کیتهای اندازه گیری این فاکتورها طبق دستورالعمل کیتها بکار برده شد. 

نتایج به صورت میانگین ± انحراف معیار (SD) ارائه گردید. برای تجزیه و تحلیل از نرم افزار SPSS و آزمون T-test استفاده شد و نمودارها توسط Graph Pad Prism ترسیم شدند. در نهایت P-value <0.05 معنی دار در نظر گرفته شد.

نتایج        

سطح LtB4 گروه آسمی (231±38 ng/ml) به طور قابل توجهی (P<05/0) نسبت به گروه سالم (118±19 ng/ml) افزایش یافت. تیمارها با هلیکوباکتر و HP-NAP منجر به کاهش معنی‌دار (P>05/0) در سطوح LtB4 درگروه تیمار با هلیکوباکتر پیلوری(201±23 ng/ml)  و HP-NAP (197±30 ng/ml)  نسبت به گروه آسمی نشد (نمودار1).

سطح LtC4 گروه آسمی(109±21 ng/ml)  به طور قابل توجهی (P<05/0) نسبت به گروه سالم (ng/ml 51±11) افزایش یافت. در حالی که تیمارها با هلیکوباکتر و HP-NAP منجر به کاهش معنی‌دار (P>05/0) در سطوح LtC4 درگروه تیمار با هلیکوباکتر پیلوری(117±19 ng/ml)  و HP-NAP  (103±11 ng/ml)  نسبت به گروه آسمی نشد (نمودار2).

 اگرچه آسم آلرژیک اغلب همراه با پاسخ های Th-2است، مشاهده شده که تعدادی از بیماران مبتلا به آسم شدید ترکیبی از پاسخ های سلولی Th-2 و Th-17 را در راه های هوایی خود نشان می دهند (27) و نیز مشخص شده که سلول های Th-l7 و T-regs نیز به طور قابل توجهی در پاتوژنز آسم دخیل هستند(30-28). عوامل زیادی، مانند Th-2، Th-17، و حتی نفوذ ائوزینوفیل/نوتروفیل، در برخی از مدل های آسم دیده شده است (17-15). این دیدگاه که آسم ائوزینوفیلیک یک اختلال انحصاری Th-2 و آسم نوتروفیلی یک اختلال انحصاری Th-17 است، خیلی دقیق نمی باشد (18). بطوری که مشخص شده که عوامل التهابی Th-2 و Th-17 در آسم یکدیگر را تنظیم می کنند (19). و Th-1/Th-2 و Th-l7/T-regs و سایتوکاین های مختلف آنها یک شبکه تعاملی بسیار پیچیده را تشکیل می دهند ( 30 ، 29. (

 هلیکوباکتر پیلوری می تواند باعث تغییرات مزمن ایمونوپاتولوژیک در معده و نامتوازن شدن میکروبیوتای دستگاه گوارش (دیس باکتریوز) شده و تنظیم عملکرد ایمنی ریه را تقویت کند. همچنین هلیکوباکتر پیلوری از ایجاد آسم توسط DCها، T-regs  و غیره جلوگیری می کند، که این با نظریه محور روده-ریه مطابقت دارد (33-31)زمانی که فردی به هلیکوباکتر پیلوری آلوده می شود، T-regs در مخاط معده بسیار فعال است و ترشح IL-10 توسط T-regs در خون محیطی به طور قابل توجهی بیشتر از سلول های Th-1 اختصاصی هلیکوباکتر پیلوری می باشد. هنگامی که یک واکنش T-regs قوی بوجود آید غلظت IgE کل و IgE مخصوص آلرژن کم می شود بطوری که سرکوب IL-10 می تواند به طور قابل توجهی مقدار IgE را در مدل های حیوانی افزایش دهد. بنابراین، همرسانی IL-10 و T-regs در پیشگیری از آلرژی ها در هنگام حضور هلیکوباکتر پیلوری ممکن است نقش داشته باشد (34).کیبورز و همکاران مدل‌های آزمایشی موش‌های C57BL/6 با ایجاد التهاب راه‌های هوایی بوسیله گرد و غبار خانگی و اوالبومین و ویروس آنفولانزای A و عفونت با سیتروباکتر رودنتیوم را ایجاد کردند وپس از قرار دادن موشها در معرض عصاره هلیکوباکتر پیلوری و سیتوتوکسین واکوئل کننده ایمنی آن در مرحله پری ناتال، مشخص شد که اینها می توانند عملکردهای محافظتی قوی را در برابر آسم آلرژیک نه تنها در فرزندان نسل اول بلکه در نسل دوم نیز بوجود آورند. تنوع و ترکیب میکروفلور دستگاه گوارش به مقدار زیادی تحت تاثیر، قرار گرفتن در معرض هلیکوباکتر پیلوری می باشد. پس قرار گرفتن در معرض هلیکوباکتر پیلوری نه تنها می تواند بر روی ناقلین بلکه در نسل های بعدی نیز موثر واقع شود. مواد مغذی مادر، قرار گرفتن در معرض میکروارگانیسم ها، سیگار و سایر عوامل محیطی نیز بر شکل گیری سیستم ایمنی در جنین از طریق روش اپی ژنتیکی تأثیر می گذارد (35) دندرتیک سل ها (DC) به سلول‌های T اولیه کمک می‌کنند تا به سلول‌های Th-2  یا به T-regs تمایز یابند (37-36) مشخص شده که هلیکوباکتر پیلوری می تواند DCها را برای ارتقای تحمل ایمنی و افزایش اثر محافظتی در برابر آسم آلرژیک هدف قرار داده و این اثر بسیار وابسته به T-regs است .(38)

نتیجه‌گیری:

  با توجه به مطالعات قبلی و یافته های ما در این پژوهش می توان اینگونه نتیجه گرفت که با توجه به اثرات قوی تر  HP-NAP نسبت به خود هلیکو باکتر شاید بتوان از آن به عنوان یک تنظیم کننده برای لکوترینهای C4, B4  استفاده و با کاهش آنها در نهایت از التهاب مجاری هوایی منجر به آسم آلرژیک جلوگیری کرد و امیدوار بود تا بتوان از HP-NAP  بعنوان یک عامل درمانی مفید برای پیشگیری و درمان آسم آلرژیک استفاده شود.

Investigation of the effect of Helicobacter pylori neutrophil-activating protein on leukotriene C4, B4 levels and airway inflammation in an allergic asthma mouse model.

Abstract

Background & Aim: Helicobacter pylori neutrophil-activating protein (HP-NAP) acts as an immunogen to elicit antigenic responses or an immunomodulator to regulate T-cell immunity. This protein can lead to a Th2 response, which may result in an allergic response or a Th1 response. It is also likely to affect the production of leukotrienes C4, B4 and finally asthma. The objective of this study is to examine the impact of HP-NAP on the concentration of leukotrienes C4 and B4 in an allergic asthma model in mice and to compare this with the effects of Helicobacter pylori.

Materials & Methods: Firstly, the Helicobacter pylori standard strain and HP-NAP peptide were prepared in accordance with the methodology outlined in a previous study. Subsequently, 40 adult Blab c 6 to 8-week-old male mice were prepared for modelling and developing allergic asthma and were divided into four groups of 10. These mice were selected for further study to investigate the changes in the aforementioned leukotrienes.

Results: demonstrated that the level of leukotrienes C4 and B4, as measured using the ELISA method, was significantly elevated in the asthmatic group compared to the healthy group. While the treatments with Helicobacter and HP-NAP did not result in a significant decrease in the levels of C4 and B4 compared to the asthmatic group,

Conclusion: In light of the more pronounced effects of HP-NAP compared to Helicobacter itself in reducing the level of C4 and B4 leukotrienes, HP-NAP was able to reduce the level of C4 and B4 leukotrienes in asthmatic animals. However, this reduction was not statistically significant when compared to the asthmatic group. Therefore, further studies are recommended.
