Abstract :
As there is no integrated system to assess the performance of higher education institutions ,so this study tries to present an operational model to self assess the performance of university departments as the most important part of universities. It attempts to use the view points and experiences of Islamic Azad University faculty members which consists of seven elements: 1.goals, organization status, management and structure.2.faculty members, 3.students, 4.teaching-learning strategies, 5.educational periods and educational planning, 6.education and research possibilities and tools and 7.educated students .Each one of elements consists of some criterions, and any criterion consists of some indicators sand indexes. All of these with Alpha 0/98 prepared and distributed between faculty members of Islamic Azad University faculty members in Gilan and Mazandaran.A model presented through these criterions, indicators and indexes.
