Abstract :

Background and purpose: The aim of this study was to investigate the modification of cognitive bias towards the symptoms of smoking on cognitive bias and susceptibility to drug addiction in smoking employees.

 

Methods: The research method was a semi-experimental type with a pre-test post-test design and experimental and control groups. The statistical population of the research included the smoking male employees of Bushehr city in 2022. Inorder to sampling, first using a random method eight offices from the government offices in Bushehr selected, then through a call smoking employees were asked to declare their readiness to participate in a psychological research project. Finally, 37 men out of 83 smokers who were ready to participate in the research were selected using a simple random method and randomly assigned to  the experimental group (19 individuals) and the control group (18 individuals). The members of the experimental group participated in an eight cognitive bias modification towards smoking symptoms sessions. The data was collected by the Sinai Cognitive Behavioral Science Institute Dot Probe Task and the Ahvaz Addiction Preparation Scale. in two stages before and after the intervention, and covariance analysis was used to analyze the data.

 

Findings: The results of the research showed that the intervention aimed at modifying the cognitive bias towards the symptoms of smoking lead to a significant reduction in the cognitive bias as well as the susceptibility to addiction of the experimental group (P<0.01).

Conclusion: Considering the effectiveness of interventions aimed at modifying cognitive bias on reducing cognitive bias and reducing the tendency to use drugs, the implementation of such interventions in populations susceptible to addiction will have an effective and low-cost role in preventing addiction.

Extended Abstract

Introduction

Water Addiction to drugs is considered one of the most significant social harms that has increasingly spread worldwide (2). In Iran, in the year 2018, over 4 million individuals were regular and occasional users of drugs (4).

    Numerous factors contribute to an individual's inclination towards addiction, but persistent cigarette smoking is the primary risk factor in this context (11), to the extent that cigarettes are referred to as the gateway to substance use (13). Smokers, due to the cognitive preoccupations they have with cigarettes, unconsciously pay more attention to cigarette cues and any stimuli associated with cigarettes (19). This unconscious attention leads to the development of a form of cognitive bias known as attentional bias in smokers (20).

Today, various intervention methods are used to address cigarette addiction. Among the interventions aimed at helping individuals quit smoking, psychological interventions hold greater importance, as psychological addiction to cigarettes cannot be controlled with medication (32). Cognitive bias modification is one of the novel psychological interventions that has led to a reduction in the severity of addiction disorders (33). The effectiveness of this intervention has been examined and confirmed on various variables.

    The goal of this intervention for smokers is to intervene in the cognitive processes of attention and evaluation triggered by addiction-related cues in the environment (42). By reducing an individual's sensitivity to cigarette cues, this intervention directly and significantly influences behaviour, resulting in behavioural changes among smokers (44). Therefore, considering the prominent role of cognitive biases and the fact that these biases significantly impact the maintenance of addictive behaviours, and given the high rate of relapse indicating the lack of success of existing intervention methods, focusing on new interventions targeting the psychological dimensions of the issue appears essential. Therefore, based on the aforementioned content, the current research aims to investigate the impact of cognitive bias modification regarding cigarette cues through the exploration program on reducing bias and the readiness for addiction among smokers.

 

Materials and Methods

The current research design is of an experimental nature, utilizing the pretest-posttest control group. The statistical population of this study consisted of employees working in government offices in the city of Bushehr during the summer of 2022 . The sample size was determined based on the required sample size for experimental research by Delavar (45), resulting in a total of 37 smokers aged between 28 and 50 years. These participants were randomly assigned to two groups: the experimental group (19 individuals) and the control group (18 individuals).

    All participants in the study took part in the pretest phase, where they completed the Dot Probe Task program and the Ahvaz Addiction Preparation Scale. Subsequently, the experimental group underwent 8 sessions of cognitive bias modification intervention. At the end of the study, both the experimental and control groups completed the Ahvaz Addiction Preparation Scale and the cognitive bias assessment using the Dot Probe Task program again. Their scores were recorded as post-test scores.

 

Findings

The findings obtained from the demographic variables indicated that none of them had a significant relationship with the research variables. The assumptions of normal distribution of variables, homogeneity of variances, and homogeneity of regression slopes were met in both the pretest and posttest stages. A significant difference is observed in the means of cognitive bias scores and addiction readiness (P < 0.05) in the posttest. The effect size and power of the developed intervention test are 0.208 and 0.827 for the cognitive bias variable, and 0.537 and 1 for addiction readiness, respectively.

 

 

 

Discussion

According to farmers, the main way to reduce water conflict has been to involve farmers in managing water and negotiating with farmers about water. These findings show that cooperation in the water management of shared wells can reduce the intensity of water conflict. In this regard, other studies have also emphasized the importance of facilitation and free expression of ideas for resolving water conflicts. According to the main strategy used by farmers to resolve water conflict, namely the control strategy, it can be concluded that farmers pay more attention to their interests in water conflict. This could exacerbate the conflict between farmers and increase conflicts and disputes over water. Research has also shown that one of the most important factors in increasing water conflict is the selfish attitude of farmers. However, if a control strategy could not result in conflict reduction, then it can worsen the situation, especially in drought. The results showed that the lower income of farmers from agriculture, the more sense and perception of water conflict. However, even with the increase in annual income from non-agricultural activities, the perception of agricultural water conflict has not diminished.

 

Conclusion

Based on the overall research objective, the investigation of the research findings demonstrated that 8 sessions of cognitive bias modification specifically targeted at cigarette cues had significant effectiveness in reducing cognitive bias and addiction readiness among smokers. Considering the shared nature of cigarette cues and substance use cues in the environment, as well as the enticing role of these cues in influencing the propensity towards substance use, the cognitive bias modification intervention helps participants redirect their attention away from tempting cigarette and substance cues and focus on positive and neutral stimuli present in their surroundings. This intervention acts as a protective factor against substance use temptation, leading to a reduction in addiction readiness. This is supported by the current study's findings, where modifying the attentional bias of smokers led to a decrease in both cognitive bias and addiction readiness.

تاریخ دریافت: 04/06/1402 تاریخ داوری: 06/05/1403 تاریخ پذیرش: 22/05/1403	چکیده زمینه و هدف: این پژوهش با هدف بررسی اثربخشی اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار بر سوگیری شناختی و آمادگی ابتلا به اعتیاد مواد مخدر کارمندان سیگاری اجرا شد. روش­: روش پژوهش نیمه‌آزمایشی با طرح پیش‌آزمون_پس‌آزمون همراه با گروه کنترل بود. جامعه آماری شامل تمامی کارمندان مرد سیگاری شاغل در ادارات دولتی شهر بوشهر در سال 1401 بود. ابتدا با استفاده از روش تصادفی، تعداد هشت اداره از ادارات دولتی شهر بوشهر انتخاب، سپس طی فراخوانی در ادارات مذکور از کارمندان سیگاری آنها خواسته شد در صورت تمایل به شرکت در یک پروژه پژوهشی روانشناختی اعلام آمادگی نمایند. سرانجام، با استفاده از روش تصادفی ساده تعداد 37 نفر از 83 فرد سیگاری که برای شرکت در پژوهش اعلام آمادگی کرده بودند، انتخاب شدند و به روش گمارش تصادفی در دو گروه آزمایش (19 نفر ) و کنترل (18 نفر) قرار گرفتند. اعضای گروه آزمایش در برنامه 8 جلسه‌ اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار شرکت کردند. ابزار مورد استفاده در این پژوهش نسخه ایرانی آزمایه کاوش نقطه موسسه علوم رفتاری شناختی سینا و پرسش‌نامه آمادگی ابتلا به اعتیاد مواد مخدر اهواز بود که در دو مرحله پیش‌آزمون و پس‌آزمون بر روی دو گروه آزمایش و کنترل انجام شدند. برای تحلیل داده‌ها از شاخص آماری تحلیل کوواریانس استفاده شده است.  یافته­ها: نتایج تحلیل نشان داد که اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار موجب کاهش معناداری در میانگین نمرات سوگیری شناختی و آمادگی ابتلا به اعتیاد گروه آزمایش نسبت به گروه کنترل شد (01/0P<) . نتیجه­گیری: نتایج این پژوهش‌ نشان داد که مداخله اصلاح سوگیری شناختی می‌تواند به‌عنوان یک روش مفید برای کاهش سوگیری شناختی و آمادگی ابتلا به اعتیاد مواد مخدر در افراد سیگاری به کار رود.

مقدمه

اعتیاد به مواد مخدر یکی از چهار مسئله اساسی است که حیات بشری را در ابعاد اجتماعی، اقتصادی، فرهنگی و سیاسی در سراسر جهان مورد تهدید قرار داده است (1). این مسئله از جمله مهم‌ترین آسیب‌های اجتماعی بشمار می‌رود که در سراسر جهان به طور فزاینده‌ای شیوع پیدا کرده است (2). براساس گزارش سازمان بهداشت جهانی، در سال 2019 بیش از 35 میلیون نفر به اختلال مصرف مواد دچار شده اند (3). در ایران در سال 1397 بیش از 4 میلیون نفر مصرف کننده دائمی و تفننی مواد مخدر بودند (4). اختلال مصرف مواد، خطر ابتلا به بیماری‌های جدی سلامت مانند بیماری‌های قلبی عروقی، بیماری‌های تنفسی، هپاتیت، ایدز و... و در نهایت، افزایش مرگ و میر را به دنبال دارد (5). علاوه بر خطرات عمده زیستی مواد مخدر، از افسردگی (6)، اضطراب و تکانشگری (7)، اختلال در خودکنترلی و عواطف (8) و احساس اجبار در انجام یک عمل (9) به عنوان مهم‌ترین خطرات روان‌شناختی اعتیاد یاد می‌شود.

    با وجود محرز شدن آسیب‌ها و عوارض ناشی از اعتیاد هر روز بر تعداد افرادی که به این دام گرفتار می‌شوند افزوده می‌شود (10). عوامل متعددی در گرایش فرد برای ابتلا به اعتیاد نقش دارد اما مصرف دائمی سیگار خطر اصلی در این زمینه است (11) و همبستگی بالایی بین اعتیاد به مواد مخدر و مصرف سیگار وجود دارد (12). یافته‌ها هم‌چنین، نشان می‌دهد که میزان آمادگی ابتلا به اعتیاد در افراد سیگاری در مقایسه با غیرسیگاری‌ها به طور معناداری بالاتر است، به‌گونه‌ای که از سیگار به‌ عنوان مهم‌ترین پیشایند و دروازه ورود به مصرف مواد اعتیادآور خطرناک یاد می‌شود (13).

    اگرچه اثرات زیان‌بار سیگار شناخته شده است، اما ترک سیگار برای افراد سیگاری بسیار دشوار است و اکثر مداخلات مربوط به ترک سیگار به ترک کامل منجر نمی‌شود (14). عواملی مانند نقص در پوشش بيمه‌اي، عدم آگاهي از وجود مراکز ترک سيگار و کمبود گروههاي حمايتي (15)، اعتیاد به نیکوتین، لذت بردن و عادت به سیگار (16) و سوگیری نسبت به نشانه‌های سیگار (17) باعث شده است که با وجود آگاهی افراد سیگاری از آسیب‌های جدی این ماده، صرفاً 40 درصد از آن‌ها در هر سال برای ترک سیگار تلاش ‌کنند و کمتر از 20 درصد اقدام‌‌کنندگان، موفق به ترک سیگار شوند (18). افراد سیگاری به دلیل مشغله‌های ذهنی که در خصوص سیگار دارند، به‌صورت ناخودآگاه نسبت به نشانه‌های سیگار مثل تصاویر جعبه‌ها یا نخ‌های سیگار، ویترین مغازه‌های سیگار فروشی، تابلوهای تبلیغات و هرگونه محرک یادآور سیگار توجهی بیش‌تر نشان می‌دهند (19). این توجه ویژه که به‌صورت ناخودآگاه صورت می‌پذیرد، باعث شکل‌گیری نوعی از سوگیری شناختی¹ تحت عنوان سوگیری توجه در افراد سیگاری می‌شود (20). سوگیری شناختی در واقع، داشتن توجه خاص به برخی اطلاعات در فرایند پردازش اطلاعات است که منجر به ‌دقت نظر و حساسیت ویژه نسبت به برخی از محرک‌های پیرامونی و چشم‌پوشی از دیگر محرک‌ها می‌شود (21). سوگیری در پردازش‌های شناختی افراد سیگاری یک معضل جدی است که منجر به اختلال در سایر کارکردهای شناختی فرد مانند برنامه‌ریزی، حل مسئله، قضاوت و تصمیم‌گیری می‌شود (22). به عبارت دقیق‌تر، سوگیری شناختی باعث کاهش تکیه بر منابع شناختی آشکار و معتبر و تکیه بر شناخت­های نا­آشکار، مبهم و ناخودآگاه شده و احتمال خطا در قضاوت و اتخاذ تصمیمات پرخطر را افزایش می‌دهد (23).

    امروزه از روش‌های مداخله‌ای گوناگونی در جهت درمان وابستگی به سیگار استفاده می‌شود. از مهم‌ترین این مداخلات استفاده از فراورده‌های جایگزینی نیکوتین مانند آدامس نیکوتینی، اسپری نیکوتینی و برچسب‌های نیکوتینی است که داروهای خط اول در دارودرمانی به‌ حساب می‌آیند و مداخله‌ای ایمن برای جمعیت عمومی و گروههای پرخطر از جمله زنان باردار و بیماران قلبی عروقی هستند (24). این روش که بر مبنای تئوری کاهش آسیب است به مراجعه‌کننده کمک می‌کند درد را تسکین دهد و در نتیجه پرهیز از توتون و تنباکو را آسان‌تر می‌کند (25) و تاثیر دو برابری نسبت به دارونماها دارد (26). مشاوره نیز به ‌عنوان یک روش درمانی مؤثر در افراد وابسته به نیکوتین شناخته می‌شود. نقش خدمات مشاوره‌ای که به ‌صورت فردی، گروهی و تلفنی ارائه می‌شود در کمک به افراد سیگاری برای ترک تأیید شده است (27) به گونه ای که حتی یک جلسه مشاوره برای ترک سیگار به‌ویژه اگر از طرف یک پزشک باشد نرخ ترک را تا 30 درصد بالا می‌برد (28). طب سوزنی و هیپنوتیزم نیز یکی دیگر از مداخلاتی است که در برنامه‌های ترک سیگار به‌صورت مؤثری استفاده شده است (29). در هیپنوتیزم به دلیل پذیرا بودن و انعطاف‌پذیری ذهن و جسم اثرات تلقین چندین برابر بیش‌تر از زمان بیداری است (30) و متخصصان گاها با استفاده از این تکنیک توانسته‌اند به ترک عاداتی مانند سیگارکشیدن به افراد کمک کنند (31). از بین مداخلاتی که در جهت کمک به ترک افراد سیگاری انجام‌گرفته است، مداخلات روان‌شناختی از اهمیت بالاتری برخوردار است، به این دلیل که وابستگی فیزیولوژیک افراد سیگاری به نیکوتین را می‌توان با دارو کنترل کرد ولی وابستگی روان‌شناختی به سیگار با دارو قابل کنترل نیست (32).

اصلاح سوگیری شناختی² از جمله مداخلات روان‌شناختی نوینی است که با هدف ایجاد تغییر در شناخت انجام می‌شود (20) و با تغییر در نحوه پردازش، کدگذاری و اصلاح روش‌های پردازش سیستماتیک اطلاعات منجر به کاهش شدت اختلالات اعتیادی شده است (33). پژوهش‌های متعدد نشان می‌دهد که مداخلات بر مبنای اصلاح سوگیری شناختی مداخلاتی مؤثر در کمک به افراد سیگاری برای ترک بشمار می‌رود (34، 35). مداخله اصلاح سوگیری شناختی کامپیوتری شامل انواع مختلفی از پارادایم­های آموزش کامپیوتری است که بر مبنای سرعت زمان واکنش شرکت‌کنندگان به نشانه­های مربوط به اختلال به ‌صورت کنترل شده و تحت شرایط و پیش‌آمد­های احتمالی پاسخ می­دهند (36) و باعث می‌شود فرایندهای شناختی خودکار به گونه مستقیم تغییر یابد (37). اثربخشی این مداخله تاکنون بر روی متغیرهای گوناگونی بررسی و مورد تایید قرار گرفته است. نتایج مطالعه بوخچبا و همکاران (38) نشان داد مداخله اصلاح سوگیری شناختی در کاهش اضطراب اجتماعی موثر است. مطالعه طاهری فرد و همکاران (39) بیانگر اثربخشی تعدیل سوگیری توجه بر کاهش عود مصرف‌کنندگان مواد مخدر بود. یافته‌های پژوهش معینی و همکاران (40) نشان داد مداخله اصلاح سوگیری شناختی در پرسنل درمانی بیمارستان، سبک دلبستگی اضطرابی و خودافشایی را کاهش و هم‌چنین، همدلی و انعطاف‌پذیری شناختی را در آنان افزایش می‌دهد. کرونر وهمکاران (41) در پژوهش خود نشان دادند اصلاح سوگیری شناختی باعث کاهش هیجانات منفی در مبتلایان به اختلال روانی استرس پس از سانحه³ می‌شود.

    در افراد سیگاری هدف از این مداخله اصلاح سوگیری شناختی، دخالت در فرایند­های شناختی توجه و ارزیابی ناشی از نشانه‌های مربوط به اعتیاد در محیط است (42). در مداخلات اصلاح سوگیری شناختی در افراد سیگاری توجه فرد به صورت تعمدی از محرک‌های مربوط به سیگار دور می‌شود و در معرض محرک‌های خنثی قرار می‌گیرد (43). آزمایه کاوش نقطه (دات پروب)⁴ آزمونی کاربردی بر مبنای اصلاح سوگیری شناختی است که در آن سوگیری در توجه افراد سیگاری سنجیده می‌شود و در بخش اصلاح، سوگیری توجهی فرد به نشانه سیگار با انتخاب اجباری محرک خنثی بجای محرک مربوط به مرور کاهش می‌یابد. این مداخله با کاهش دادن حساسیت فرد نسبت به نشانه‌های سیگار تأثیر مستقیم و معناداری بر رفتار دارد و منجر به تغییر در عملکرد رفتاری افراد سیگاری می‌شود (44).

لذا، با توجه به نقش برجسته سوگیری‌های شناختی و از آنجایی که این سوگیری­ها تاثیر بسزایی در حفظ رفتارهای اعتیادآور دارد و با توجه به درصد بالای بازگشت که بیانگر عدم‌موفقیت روش‌های مداخله‌ای موجود هست، تمرکز بر روی مداخلات جدید که ابعاد روان‌شناختی موضوع را هدف قرار می‌دهند ضروری به نظر می‌رسد. اهمیت پیشگیری از ابتلا به اعتیاد و نیاز اساسی جامعه به مداخلات پیشگیری‌محور و هم‌چنین، با توجه به مکانیسم برنامه اصلاح سوگیری شناختی در کنترل توجه افراد مستعد اعتیاد و به دلیل کمبود پژوهش‌های کاربردی داخلی مبنی بر اثربخشی این روش مداخله در حوزه اعتیاد این نیاز احساس می‌شد که مطالعه‌ای کاربردی اثربخشی این روش مداخله را بررسی کند. بنابراین، با توجه به مطالب ذکر شده این پژوهش به ‌منظور بررسی تأثیر اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار به‌وسیله برنامه کاوش نقطه بر کاهش سوگیری و هم‌چنین، آمادگی ابتلا به اعتیاد افراد سیگاری اجرا شد.

 

مواد و روش­ها

این پژوهش به منظور بررسی اثربخشی برنامه مداخله‌ای اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار بر آمادگی ابتلا به اعتیاد افراد سیگاری، با استفاده از طرح نیمه‌آزمایشی پیش‌آزمون-پس‌آزمون با گروه کنترل اجرا شد. جامعه آماری این پژوهش شامل کارمندان مرد سیگاری شاغل در ادارات دولتی شهر بوشهر در تابستان سال 1401 بود. نمونه آماری بر اساس نمونه مورد نیاز در پژوهش‌های آزمایشی (45) تعداد 40 فرد سیگاری بودند. برای انتخاب نمونه در ابتدا با استفاده از روش تصادفی، تعداد هشت اداره از لیست ادارات دولتی شهر بوشهر انتخاب، سپس طی فراخوانی در ادارات مذکور از کارمندان خواسته شد درصورت تمایل به شرکت در یک پروژه پژوهشی روانشناختی اعلام آمادگی نمایند. سرانجام، تعداد 40 نفر از 83 فرد سیگاری که برای شرکت در پژوهش اعلام آمادگی کرده بودند، با استفاده از روش تصادفی ساده انتخاب و به روش گمارش تصادفی در دو گروه آزمایش و کنترل (20 نفر آزمایش و 20 نفر کنترل) قرار گرفتند. در طی اجرای پژوهش، یک نفر از گروه آزمایش از ادامه مشارکت در پژوهش خودداری کرده و دو نفر از گروه کنترل نیز در جلسه تکمیل پرسش‌نامه‌های پس‌آزمون شرکت نکردند، بنابراین، نمونه آماری شامل 37 فرد سیگاری (19 نفر در گروه آزمایش و 18 نفر در گروه کنترل) با دامنه سنی 28 تا 50 سال بودند. معیارهایی همچون کشیدن حداقل 5 نخ سیگار در طول روز، عدم‌مصرف داروهای روان‌گردان و تحت درمان بیماری جسمانی یا روانی نبودن و امضاء فرم رضایت آگاهانه به‌ عنوان معیارهای ورود به پژوهش و عدم‌همکاری در هر کدام از مراحل پژوهش، مصرف داروهای روانپزشکی و مصرف مواد و عدم‌شرکت در بیش از دو جلسه از مراحل اصلاح سوگیری به‌عنوان معیار خروج از پژوهش لحاظ شد. تمامی افراد حاضر در پژوهش (گروه آزمایش و کنترل) پس از جلسه توجیهی درباره نحوه اجرای پژوهش به‌وسیله آزمایه کاوش نقطه و پرسش‌نامه آمادگی ابتلا به اعتیاد مواد در پیش‌آزمون شرکت کردند. سپس گروه آزمایش 8 جلسه مداخله اصلاح سوگیری شناختی را دریافت کردند. پس از اجرای طرح مداخله‌ای برای گروه آزمایش، اعضای هر دو گروه، مجددا پرسش‌نامه آمادگی ابتلا به اعتیاد را تکمیل کرده و سوگیری شناختی آنها نیز با آزمایه کاوش نقطه مورد سنجش قرار گرفت. در نهایت، گروه کنترل نیز در مورد نتایج مثبت این مداخله توجیه شدند و به همه آنان آموزش استفاده از این نرم‌افزار و نحوه اثرگذاری آن داده شد.

 

ابزار گردآوری داده‌ها

پرسش‌نامه آمادگی به اعتیاد مواد مخدر اهواز: این مقیاس توسط زرگر (1385) با هدف ارزیابی آمادگی افراد برای ابتلا به اعتیاد به مواد مخدر متفاوت از سیگار ساخته شده است و دارای 36 آیتم به‌اضافه 5 آیتم دروغ‌سنج می‌باشد (46). نمره گذاری پرسش‌نامه آمادگی به اعتیاد بر اساس لیکرت چهار گزینه‌ای از صفر (کاملاً مخالفم) تا 3 (کاملاً موافقم) می باشد و دو عامل آمادگی فعال (27 آیتم) و آمادگی منفعل (9 آیتم) برای اعتیاد را می‌سنجد. آمادگی فعال مربوط به رفتارهای ضداجتماعی، میل به مصرف مواد، نگرش مثبت به مواد و هیجان‌خواهی است و عامل آمادگی منفعل نیز شامل ویژگی‌هایی مانند ناتوانی در ابراز وجود و افسردگی می‌باشد. دامنه نمره در این پرسش‌نامه از 0 تا 108 می‌باشد که نمره بین 0 تا 36 بیانگر آمادگی پایین برای اعتیاد، نمره 36 تا 54 بیانگر آمادگی متوسط برای اعتیاد، و نمره بالاتر از 54 بیانگر آمادگی بالا برای ابتلا به اعتیاد است. زرگر، نجاریان و نعامی روایی سازه این مقیاس را  از راه همبسته کردن آن با مقیاس 25 ماده‌ای فهرست بالینی علائم بالینی 45/0 محاسبه کرده‌اند که معنا‌دار می‌باشد. هم‌چنین، اعتبار مقیاس با روش آلفای کرونباخ 90/0 محاسبه شده که در حد مطلوب است (47).

 آزمایه کاوش نقطه (دات پروب): نسخه اصلی این ابزار در سال 1986 توسط مکلئود، ماتیوس و تاتا (48) برای اندازه گیری و اصلاح توجه گزینشی و سوگیری‌های شناختی طراحی شده و نسخه ایرانی آن در مؤسسه تحقیقات علوم رفتاری شناختی سینا ساخته شده است (37). اساس کار در این آزمون رایانه‌ای بدین شکل است که دو محرک (خنثی و هدف) هم‌زمان بر روی یک صفحه نمایش ظاهر شده و زمان واکنش آزمودنی بیانگر فرایند ترجیح یکی از محرک‌ها نسبت به دیگری است، هرچه زمان واکنش به یک محرک کمتر باشد نشانگر توجه گزینشی و سوگیری بیش‌تر به آن محرک است. برای اجرای این آزمون، آزمودنی در موقعیت فاصله‌ای 60 سانتیمتری از صفحه نمایش رایانه قرار می‌گیرد. ابتدا آزمودنی به علامت (+) که در وسط صفحه‌ نمایش به مدت 500 هزارم ثانیه ظاهر می‌شود نگاه می‌کند، در مرحله بعد علامت تثبیت (+) ناپدید می‌شود و دو تصویر که شامل یک تصویر هدف (به عنوان نمونه، در این پژوهش جاسیگاری) و یک تصویر خنثی (به عنوان نمونه، نعلبکی) است برای مدت 5/1 ثانیه در دو سمت صفحه نمایش ظاهر می‌شود. سپس تصاویر ناپدید شده و یک نقطه (•) جانشین یکی از تصاویر می‌شود. آزمودنی در این تکلیف باید به‌ محض تشخیص نقطه بر روی صفحه نمایش به‌وسیله کلیدهای جهت‌نما بر روی صفحه‌کلید جهت درست نقطه ظاهر شده را مشخص کند. میزان سوگیری شناختی بر اساس کسر مدت زمان واکنش به محرک هدف از مدت زمان واکنش به محرک خنثی محاسبه شده و به میلی ثانیه گزارش می‌شود. در برنامه مداخله‌ای، برای کاهش سوگیری توجه فرد نسبت به نشانه‌های سیگار، در بیش از 80 درصد از موارد نقطه به جای محرک‌های خنثی ظاهر می‌شود. به ‌این ‌ترتیب، فرد به‌ مرور با حجم بیش‌تری از ترجیح محرک‌های خنثی نسبت به محرک هدف مواجه می‌شود. پایایی این آزمون در ایران در سال 1388 توسط دهقانی و همکاران (49) بررسی شد که به روش آلفای کرونباخ بالای 90/0 بوده است. محاسبه روایی ابزار نیز توسط طاهری‌فرد و همکاران با بررسی نمره سوگیری آزمودنی‌ها با نمرات پرسش‌نامه افسردگی و اضطراب به روش آلفای کرونباخ 82/0 محاسبه شد (39).

 

نتایج

یافته‌های پژوهش در دو سطح توصیفی (میانگین و انحراف استاندارد) و استنباطی (تحلیل کواریانس) تجزیه و تحلیل شد. اطلاعات جمعیت‌شناختی سن و سطح تحصیلات نمونه‌های شرکت کننده در پژوهش شامل تعداد 37 کارمند با میانگین سنی 3/41 در بازه سنی 28 تا 50 سال با انحراف معیار 11/6 بود که از این تعداد 6/21 درصد (8 نفر) مجرد و 4/78 درصد (29 نفر) متاهل بودند. از نظر تحصیلات 3/24 درصد (9 نفر) کاردانی، 4/59 درصد (22 نفر) کارشناسی و 3/16 درصد (6 نفر) دارای مدرک کارشناسی ارشد بودند.

    یافته‌های بدست آمده از متغیرهای جمعیت‌شناختی نشان داد هیچ یک رابطه معناداری با متغیرهای پژوهش ندارد و همه سطوح معنی‌داری از 05/0 بیش‌تر بود. بنابراین، نیازی به کنترل این متغیرهای در پژوهش نبود. یافته‌های توصیفی سوگیری شناختی و آمادگی ابتلا به اعتیاد در جدول 1 ارائه شده است.

        باتوجه ‌به نتایج جدول 1 مشاهده می‌شود که میانگین نمرات برای متغیرهای سوگیری شناختی و آمادگی ابتلا به اعتیاد در دوره پس‌آزمون نسبت به پیش‌آزمون در گروه‌ آزمایش تغییر قابل توجهی داشته است اما در گروه کنترل تفاوت محسوسی در بین متغیرها در پیش‌آزمون و پس‌آزمون دیده نمی‌شود. هم‌چنین، به‌منظور بررسی توزیع نرمال متغیرها از آزمون کولموگروف- اسمیرنف استفاده شد، سطح معناداری بیش‌تر از 05/0 بود که حاکی از نرمال بودن توزیع متغیرها است. سطح معناداری در آزمون لوین برای متغیرهای سوگیری شناختی و آمادگی ابتلا به اعتیاد بیش‌تر از 05/0 بود بنابراین، فرض همگنی واریانس‌ها در پیش‌آزمون و پس‌آزمون رعایت شده است. هم‌چنین، سطح معناداری در آزمون شیب رگرسیون بیش‌تر از 05/0 بود بنابراین، فرض همگنی شیب رگرسیون نیز تایید شد. در نتیجه همه پیش فرض‌های لازم برای اجرای تحلیل کواریانس رعایت شده‌اند و داده‌ها قابلیت اجرای این آزمون را دارند. در جدول 2 تحلیل کواریانس متغیرهای سوگیری شناختی و آمادگی ابتلا به اعتیاد دو گروه در مرحله پس‌آزمون گزارش شده است.

    با توجه نتایج نشان داده شده در جدول شماره 2 تفاوت معناداری بین میانگین نمرات سوگیری شناختی و آمادگی ابتلا به اعتیاد (05/0P<) در پس‌آزمون مشاهده می‌شود. اندازه اثر و توان آزمون مداخله تدوین شده به ترتیب برای متغیر سوگیری شناختی 208/0 و 827/0 و برای آمادگی ابتلا به اعتیاد 537/0 و 1 می‌باشد.

    توجه به تفاوت میانگین گروه در پیش‌آزمون و پس‌آزمون و نتایج آزمون تحلیل کواریانس جدول 2 نشان می‌دهد که 8 جلسه اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار در گروه آزمایش باعث شده است سوگیری شناختی و آمادگی ابتلا به اعتیاد این گروه به طور معناداری کاهش یابد. بنابراین، مداخله اصلاح سوگیری شناختی مبتنی بر دات پروب ابزاری مناسب جهت کاهش سوگیری شناختی و آمادگی ابتلا به اعتیاد در افراد سیگاری بود.

 

بحث و نتیجه­گیری

هدف این پژوهش بررسی اثربخشی مداخله اصلاح سوگیری شناختی نسبت به نشانه‌های سیگار بر سوگیری شناختی و آمادگی ابتلا به اعتیاد افراد سیگاری بود. اولین یافته این پژوهش نشان داد 8 جلسه اصلاح سوگیری شناختی به‌وسیله برنامه کاوش نقطه باعث کاهش سوگیری شناختی افراد سیگاری می‌شود که با یافته‌های نیک‌بخت (50)، اتوود و همکاران (51) و ارسنجانی و همکاران (52) از نظر کارایی برنامه اصلاح سوگیری شناختی در کاهش سوگیری‌های شناختی همسو بود. در تبیین این یافته می‌توان بیان کرد که افراد سیگاری در مقایسه با سایر افراد به واسطه نگرانی و دل‌مشغولی که نسبت به نشانه‌های سیگار دارند، حساسیت بیش‌تری به این نشانه‌ها در محیط دارند. این حساسیت و گوش به زنگی که باعث توجه گزینشی و خودکار این گروه به نشانه‌های سیگار می‌شود تحت عنوان سوگیری شناخته می‌شود (53). برنامه کاوش نقطه که بر مبنای اصلاح سوگیری شناختی ساخته شده است محیطی را فراهم می‌کند که در آن به مرور از توجه گزینشی فرد به نشانه‌های سیگار کاسته می‌شود (37). در تنظیمات این برنامه بر روی بخش اصلاح سوگیری در بیش از 80 درصد موارد دایره در پشت محرک خنثی قرار می‌گیرد و این باعث می‌شود فرد با کوشش‌های بیش‌تری مواجه شود که کلید جهت نمای تصاویر خنثی را فشار دهد. بنابراین، پس از چند جلسه مداخله، توجه گزینشی آزمودنی به مرور از تصاویر هدف برداشته می‌شود. همانطور که در این پژوهش پس از 8 جلسه مداخله اصلاح سوگیری، توجه گزینشی به نشانه‌های سیگار کاهش داشت.

    یافته دیگر این پژوهش نشان داد که مداخله اصلاح سوگیری شناختی به وسیله برنامه کاوش نقطه باعث کاهش نمره آمادگی ابتلا به اعتیاد در گروه آزمایش شد. این پژوهش از نظر کارایی این نوع مداخله همسو است با مطالعات طاهری‌فرد و همکاران (39)، منتظری و همکاران (54)؛ ویرز و همکاران (42)؛ بوفو و همکاران (44)؛ بوخچبا و همکاران (38) و جونز و شارپ (20). نتایج این پژوهش تایید کننده این موضوع است که فاکتورهای شناختی توجه، عاملی مهم در آمادگی ابتلا به اعتیاد است. آمادگی ابتلا به اعتیاد به این نکته اشاره دارد که برخی افراد در صورت مواجهه با عوامل ‌زمینه‌ای به احتمال بیش‌تری گرفتار اعتیاد می‌شوند (55). افراد سیگاری از جمله افرادی هستند که از آمادگی ابتلا به اعتیاد بالایی برخوردارند (56) به‌گونه‌ای که از سیگار به عنوان دروازه ورود به اعتیاد یاد می‌شود (13). به همین دلیل بررسی عوامل ‌زمینه‌ای که بر آمادگی اعتیاد به‌ویژه در گروههای پرخطر تاثیر دارد امری ضروریست (57). یکی از مصادیق این عوامل زمینه‌ای، سوگیری توجه به نشانه‌های سیگار در افراد سیگاری است. توجه گزینشی و سوگیرانه به نشانه‌های سیگار موجود در محیط باعث برانگیختگی رفتار سیگارکشیدن در افراد سیگاری می‌شود (58) و این سوگیری به‌تدریج از راه فرایند شرطی شدن، علاوه بر سیگار به نشانه‌های مواد و نشانه‌های مصرف نیز سرایت پیدا می‌کند (59). بنابراین، با توجه به مشترک بودن نشانه‌های سیگار و مصرف مواد در محیط و با عنایت ‌به نقش وسوسه‌انگیز این نشانه‌ها در گرایش به مصرف مواد، مداخله اصلاح سوگیری شناختی که از آن به‌عنوان واکسن شناختی نیز یاد می‌شود (60)  به آزمودنی‌ها کمک می‌کند تا توجه‌شان را از نشانه‌های وسوسه‌کننده سیگار و مواد بازگیری کرده و به محرک‌های مثبت و خنثی موجود در محیط متمرکز شوند. کسب توانایی کنترل توجه که در پی عدم‌توجه به نشانه‌های سیگار و مواد در طی چند جلسه اصلاح سوگیری شناختی در فرد ایجاد می‌شود و به زندگی روزمره تعمیم پیدا می‌کند عاملی محافظت کننده در برابر وسوسه‌ مصرف مواد است و میل به مصرف و در نتیجه آمادگی ابتلا به اعتیاد را کاهش می‌دهد. بنابراین، سوگیری توجه که نقش اساسی در گرایش به مواد (61) و شکل‌گیری رفتارهای اعتیادی افراد سیگاری دارد (62) به ‌مرور زمان در یک فرایند معکوس، تحت ‌تأثیر اصلاح سوگیری شناختی، تعدیل و حذف می‌شوند (44) و می‌تواند نقش پیشگیرانه مؤثری در ابتلا به اعتیاد داشته باشد. آن‌چنان‌که در این پژوهش نیز با تعدیل سوگیری توجهی افراد سیگاری، ضمن کاهش سوگیری شناختی، آمادگی ابتلا به اعتیاد نیز کاهش یافت.

    در نهایت، این پژوهش نیز مانند سایر پژوهش‌ها عاری از محدودیت نیست. نمونه‌های مورد بررسی در این پژوهش محدود، صرفا مرد و دارای سطح تحصیلات کاردانی به بالا بودند بنابراین، تعمیم نتایج آن به جامعه زنان و سایر سطوح جامعه باید با احتیاط صورت پذیرد. در همین راستا پیشنهاد می‌شود این پژوهش به صورت وسیعتر در جامعه زنان و تمامی سطوح تحصیلی نیز انجام شود. هم‌چنین، پیشنهاد می‌شود تاثیر این نوع مداخله در ترکیب با سایر مداخلات ترک سیگار بررسی و در صورت موفقیت در درمان و ترک افراد سیگاری به عنوان یک روش درمانی ارزان قیمت و در دسترس مورد استفاده قرار گیرد. با توجه به موفقیت این مداخله در کاهش آمادگی به اعتیاد افراد سیگاری پیشنهاد می‌شود از این روش در کاهش آمادگی اعتیاد سایر گروههای پرخطر اعتیاد نیز استفاده شود.

در این پژوهش موازین اخلاقی رعایت و به آزمودنی‌ها تضمین داده شد که داده‌های شخصی و مباحث مطرح شده محرمانه خواهد بود و اصل رازداری کامل در نظر گرفته شده است. هم‌چنین، تمام آزمودنی‌ها آگاهانه و با رضایت کامل در پژوهش شرکت کردند.
