Objective: The objective of the current study was to develop and standardize a gym safety questionnaire based on the RP (Risk Priority) method. Methodology: Initially, a 70-item questionnaire was designed through a review of theoretical foundations and consultations with gym managers and academic experts in the field of sports facility safety. During the face and content validity assessment stage, 9 items were excluded. Subsequently, the gym safety assessment questionnaire, focusing on two dimensions—risk severity and risk probability—was completed by 183 gym managers in Karaj. Findings: Exploratory Factor Analysis (EFA) identified eight components: health facilities safety, accessibility, space safety, equipment safety, occupational safety, hygiene, education-based safety, and facility safety. Confirmatory Factor Analysis (CFA) conducted via Smart-PLS software reported favorable validity and reliability for the instrument. Conclusion: Consequently, the Gym Safety Questionnaire can be utilized as a reliable checklist for assessing risk levels in fitness centers.
