purpouse: Problematic, inefficient, dysfunctional organizations that behave and act like addicts and have an uncontrollable and chaotic work environment are considered a hologram. These Addictive behaviors of the organization threaten its survival. Previously no comprehensive study has been done in this field therefore the aim of the present study is to design a model for exiting universities from addict organizations with a holographic approach. Method: This qualitative study was conducted by using the foundational data theory. The statistical population included 13 experts in the field of education and improvement of human resources of fars university medical sciences who were selected through targeted sampling. In order to collect data, semi-structured interviews were used and simultaneous analysis of the interviews was done by using three stages of open, central and selective coding. Finding: The results of the research consisted of identifying the holographic reasons of organizations, which included causal factors (9 components), background conditions (10 components), intervening conditions (14 components), strategy (8 components) and positive results (8 components). Conclusion: The final result of this study showed that awareness of holographic factors can help to implement strategies and mechanisms to overcome the negative consequences of the addicted organization and increase the general index of health and productivity.
