Comparison of inguinal hernia surgery results in children with and without high ligation of hernia sac with consideration of conditions and necessity

Abstract :

Background:During surgery of inguinal hernia in pediatrics,the rupture of hernia sac is probable, also the suture of sac may be unfastened by a minimal tension and result in retraction of sac into the abdominal cavity. religating the retracted sac, into the inguinal internal ring, can cause injury to the vas and vesseles. Aim:The purpose of this study was comparison of inguinal hernia surgery results in children with and without high ligation of hernia sac with consideration of conditions and necessity. method:This cross sectional study was done between 1/1384 and 9/1387(shamsi) in pediatric surgery centers of Mashhad (Iran). Amongst  287 children who undergone hernia repair surgery by a single surgeon, 100 patients were return for regular follow-ups. Of these patients, ligation of hernia sac was done on 76 cases. In the rest of patients (24 cases) hernia was repaired without ligation Results:  In 1-3 years follow-up there were no recurrences or complications ,which need intervention, in any group. Also there were no complication during surgery such as bulging viscera out of  surgical inscision after retraction of missed hernia sac. Conclusion:  According to absence of recurrences or complications ,which need intervention, in any groups it can be concluded that when there is no possibility of  easy ligation of hernia sac without any damage to cord we can avoid the high ligation of processus vaginalis. Anyway there is need for other studies with more patients to confirm the safety of this recommendation.
