Comparison of the effect of 17α- Hydroxy Progesterone caproate in prevention of preterm labor in twin pregnancies

Abstract :
Background and objective: There are indications that prophylactic administration of 17 alpha- hydroxyprogesterone caproate (17 alpha- OHP-C) could be beneficial in the treatment of women at risk for preterm delivery. Since twin pregnancy is commonly associated with prematurity the aim of this study was to evaluate the effect of 17α- Hydroxy Progesterone caproate in prevention of preterm labor in twin pregnancies. Methods: 66 women with twin pregnancy (33 in control and 33 in case group) were enrolled. 33 women with 16 to 20 weeks’ gestation were scheduled to receive randomly a single intramuscular injection of 250 mg of 17- OHPC every week until 30 weeks of gestation. Women in control group received no drugs. Two groups were similar considering meternal age and number of past pregnancies. Results: Mean age of women in control and progestron groups were 27.4± 3.9 and 26.5± 4.4 years. There was no significant differences between two groups in gestational age, admission in neonatal intensive care unit (NICU), presentation of fetus, birth weight of neonatal and meternal pain and bleeding. Administration of 17α- OHP-C caused no significant decrease in preterm labor (PTL). Although there was no significant differences in neonatal apgar at 5 minutes of birth, apgar at 1 minute of birth was significantly higher in case group (P=0.011). Conclusion: The administration of 17- alpha- OHP-C thus seems ineffective in the prevention of prematurity risks associated with twin pregnancy.
